COVID-19 Outbreak Among Farmworkers — Okanogan County, Washington, May–August 2020

農業労働者におけるCOVID-19アウトブレイク ― ワシントン州オウカノガン郡、2020年5～8月

2020年夏、ワシントン州オウカノガン郡にてCOVID-19の原因ウイルスであるSARS-CoV-2の市中感染が増加した。5～8月の果樹栽培従事者での大規模アウトブレイクなど、農業環境において複数のCOVID-19アウトブレイクが発生した。オウカノガン郡公衆衛生局とワシントン州保健局は1回限りの現地スクリーニング調査を開始し、2020年8月の時点ですべての果樹園および貯蔵庫の労働者を対象にSARS-CoV-2感染リスクを評価した。2020年5月下旬にオウカノガン郡の果物生産者が症候性の従業員に対しSARS-CoV-2検査を勧め始め、8月下旬には、全従業員に対し単回のスクリーニング検査を行った。果樹園生産者の従業員4,955名(果樹園従業員3,708名、貯蔵庫従業員1,247名)のうち、3,739名(75％、果樹園従業員3,013名、貯蔵庫従業員726名)を今回の分析対象とした。SARS-CoV-2検査が陽性(確診例)であったのは348名[9％、それぞれ178名(6%)、170名(23%)]、COVID-19疑い例(症状はあるが検査を拒否)は71名(2％)であった。果樹園従業員3,013名のうち、628名(21％)は地域の住宅に居住、2,385名(79％)は従業員用住宅に居住していた。確診例178名のうち、158名は5～8月に症状のある人を対象とした検査(症候性検査)、20名は8月のスクリーニング検査で判明し、71名は疑い例であった。地域に居住する628名中、確診例は76例(12％)、疑い例12例であり、従業員用住宅に居住する2,385名中、確診例は102例(4％)、疑い例は59例であり、罹患率は地域に居住する従業員が従業員用住宅に居住する従業員よりも有意に高かった。貯蔵庫従業員726名では確診例170名のうち、162名は症候性検査、8名はスクリーニング検査で判明し、疑い例は認めなかった。貯蔵庫従業員を、1)果物の仕分けと梱包(304名)、2)その他の梱包(126名)、3)フォークリフト操作(86名)、4)事務職(49名)、5)その他(清掃、メンテナンス、輸送など、110名)、6)不明(51名)に分けた場合の発症率は、6％[事務職(3/49名)]～が28％[仕分けと梱包(84/304名)]とバラツキを認めた。また、果樹園従業員は主に屋外、貯蔵庫従業員は広さの異なる3カ所(A、B、C)にて作業しており、作業現場別の発症率は作業現場Aが4％(5/125名)、作業現場Bが37％(44/118名)、作業現場Cが25％(121/483名)であった。多変量回帰分析では、フォークリフト操作員と作業現場Aを基準とした場合、仕分けと梱包、その他の梱包、作業現場B、作業現場Cにて感染リスクの有意差を認め、農業環境でのSARS-CoV-2感染の予防には屋内作業時の予防対策が重要であることが示唆された。
