Emergency Department Visits for Tick Bites — United States, January 2017–December 2019
マダニ咬傷による救急外来受診 ― アメリカ、2017年1月~2019年12月
アメリカではマダニ媒介性疾患の罹患率が増加しており、報告された症例は2004年の22,000例超から2016年には48,000例倍と2倍以上に増加した。各州およびワシントンDCから報告された節足動物媒介性疾患の約95%がダニによるものであり、それらの症例の80%超をライム病が占めている。マダニに咬まれた場合、救急科(ED)を受診してダニを除去してもらい、72時間以内にライム病の予防に有効な曝露後予防治療を受ける。今回、CDCのNational Syndromic Surveillance Program(NSSP)データを用いて、2017年1月~2019年12月におけるマダニ咬傷によるED受診件数を性別、年齢別、地域別、季節別に調査した。この期間のダニ咬傷による平均年間ED受診件数は49,788件であり(ED受診10万件あたり49件)、男性が平均28,147件(同63件)、女性が21,641件(同39件)と男性に多く、年齢別では0~9歳が10,142件(同86件)と最も多かった。地域別では北東部にてもっとも多く(31,340件、同110件)、中西部(8,208件、同36件)、南西部(7,680件、同26件)と続いた。季節別の受診数は二峰性であり、4~7月と10~11月にピークがあり、4月6,282件(同77件)、5月12,263件(同144件)、6月9,972件(同124件)、10月4,476件(同51件)、11月3,136件(同37件)であった。現在、ダニ咬傷に関するサーベイランスシステムはないが、ダニ咬傷による症候群サーベイランスデータはダニの生息地を避ける、屋外でダニを寄せ付けない服装の常時着用、リスクの高い時期における定期検査の実施などの公衆衛生上の情報を適時に提供しうるものである。
