Death Certificate–Based ICD-10 Diagnosis Codes for COVID-19 Mortality Surveillance — United States, January–December 2020
COVID-19死亡サーベイランスのための死亡診断書をベースとしたICD-10診断コード ― アメリカ、2020年1月~12月
2020年にCDCに報告された死亡診断書では約375,000人の死亡がCOVID-19に起因していたが、一部の死亡は不適切にCOVID-19が寄与したものとされているという懸念が提起されている。公式な死亡診断書での国際疾病分類第10版(ICD-10)による診断の分析は、報告されたCOVID-19の死亡が過大評価されているかどうかを示すための便宜的で効率的な方法を提供する可能性がある。CDCは、2021年2月22日時点でCDCに報告されていた2020年のアメリカの死亡診断書でICD-10コードのCOVID-19(U07.1)が同時に生じていた診断記録を評価した。U07.1コードが記載された378,048人の死亡診断書のうち、計357,133人(94.5%)には他のICD-10コードがあり、20,915人(5.5%)がU07.1コードのみであった。全体で、他の診断もある357,133人の死亡証明書の97.3%(378,048人の死亡証明書の91.9%)には、妥当な一連のイベント(肺炎、呼吸不全、成人呼吸窮迫症候群、心停止、敗血症など)または重大な原因となる疾患(高血圧、糖尿病、認知症、慢性閉塞性肺疾患など)あるいはその両方の診断が同時に生じていたと記載されていた。死亡診断書のパートIにCOVID-19が記載された330,198人の死亡診断書で最も多く記載された一連のイベントのICD-10診断コードは、J18.9(肺炎、45%)およびJ96.0(急性呼吸不全、20%)で、最も多く記載された重大な原因となる疾患のICD-10コードは、I10(本態性高血圧症、18%)およびE14.9(糖尿病、10%)であった。すべての死亡診断書の75%近くが、一連のイベントが単独または重大な原因となる疾患の併存であった。この調査結果は、18~84歳の成人、男性と女性、すべての人種と民族、入院患者および外来患者または救急科で死亡した人、死因が自然死と記載された人に認められ、公式の死亡診断書を使用したアメリカでのCOVID-19死亡サーベイランスの正確さを裏付けている。包括的で信頼できる公的記録のためには、死亡診断書に併存で診断される記載の品質の高さが絶対に必要である。パンデミックが進行するにつれ、死亡診断書を記入する専門医に対する継続的なメッセージングとトレーニングは依然として重要である。正確な死亡サーベイランスは、COVID-19の原因ウイルスであるSARS-CoV-2の変異株、COVID-19ワクチン接種の影響を理解し、公衆衛生活動の指針として重要である。
