First Month of COVID-19 Vaccine Safety Monitoring — United States, December 14, 2020–January 13, 2021
COVID-19ワクチン安全性モニタリングの最初の1カ月 ― アメリカ、2020年12月14日~2021年1月13日
アメリカでは現在、新型コロナウイルス感染症(COVID-19)の2つのワクチンが使用を許可されている。FDAは、2020年12月11日にPfizer社-BioNTech社製COVID-19ワクチン、2020年12月18日にModerna社製COVID-19ワクチンの緊急使用許可(EUA)を発行し、どちらのワクチンもシリーズで2回接種される。Advisory Committee on Immunization Practicesは、2020年12月12日および2020年12月19日に、Pfizer社-BioNTech社製およびModerna社製のCOVID-19ワクチンに関する暫定勧告をそれぞれ発表し、医療従事者と長期療養施設(LTCF)の居住者に対する初回接種が推奨された。これらのワクチンの安全性モニタリングはアメリカの歴史において最も強力で包括的なものとなっており、COVID-19全国接種プログラムの初期実施段階で、自発的報告システムであるVaccine Adverse Event Reporting System(VAERS)と積極的なサーベイランスシステムであるv-safeを使用している。CDCは、ワクチン接種の最初の1カ月(2020年12月14日~2021年1月13日)の安全性データの記述的分析を実施した。この期間にアメリカでは合計13,794,904回のCOVID-19ワクチンが接種され、8,436,863回(61.2%)が女性に接種された。VAERSは、この期間中にCOVID-19ワクチン接種後の有害事象として6,994件の報告を受け、このうち6,844件(97.9%)は、LTCF居住者ではなかった。すべての報告の中で、6,354件(90.8%)が重篤ではなく、640件(9.2%)が重篤な有害事象に分類された。VAERSに報告されたワクチン接種者の年齢中央値は42歳(5~104歳)、女性の有害事象報告は5,505件であった(78.7%)。頭痛(22.4%)、倦怠感(16.5%)、浮動性めまい(16.5%)がワクチン接種後に最も多く報告された症状であり、どちらのワクチンでも認められた。アナフィラキシーの報告は62件確認され、Pfizer社-BioNTech社製ワクチンが46件(74.2%)、Moderna社製ワクチンが16件(25.8%)で、両方のワクチンの接種後におけるアナフィラキシー報告はまれであったであった(100万回の接種あたり4.5件)。合計113名の死亡がVAERSに報告され、そのうち78名(65%)がLTCF居住者であり、死亡診断書、剖検報告、医療記録、およびVAERS報告と医療従事者の臨床的記述から入手可能な情報では、COVID-19ワクチン接種と死亡との間に因果関係は示されなかった。2020年12月14日~2021年1月13日に、v-safeは少なくとも1つの調査を完遂したワクチン接種者1,602,065名が登録され、814,648名(50.8%)がPfizer社-BioNTech社製、787,417(49.2%)がModerna社製ワクチンを接種していた。ワクチンの1回目投与を接種した人における局所および全身の反応は、Pfizer社-BioNTech社製ワクチンおよびModerna社製のワクチンで同様であり、注射部位の疼痛、倦怠感、頭痛、筋肉痛、悪寒が最も多く報告された。Pfizer社-BioNTech社製ワクチンでは、2回目接種後の反応は1回目接種後よりも多く、報告された発熱および悪寒の発現率は、2回目接種後が1回目接種後よりも4倍以上高かった。これらのデータは、医療従事者とワクチン接種者に対しワクチン接種後に予想される反応の参考になる情報を提供し、不安を取り除くものである。
