COVID-19パンデミック中のメディケア受給者によるワクチン接種の低下 ― アメリカ、2020年

2020年3月13日、アメリカは新型コロナウイルス感染症(COVID-19)アウトブレイクに対し国家緊急事態を宣言し、これを受けて多くの州政府や地方自治体が屋内退避または外出禁止令を出し、屋外での不必要な活動が制限された。CDCは当初、成人における定期ワクチン接種の延期を推奨するガイドラインを発行したが、後に定期ワクチン接種の継続を推奨すると改訂している。今回、2020年1月5日～7月18日の65歳以上のメディケア受給者における4種類のワクチン[13価肺炎球菌結合型ワクチン(PCV13)、23価肺炎球菌莢膜ポリサッカライドワクチン(PPSV23)、破傷風・ジフテリア/破傷風・ジフテリア・無細胞性百日咳ワクチン(Td/Tdap)、組み換え帯状疱疹ワクチン(RZV)]の接種率を2019年1月6日～7月20日の接種率と比較した。平均分母人口は、PCV13およびPPSV23は、2019年27,194,802名、2020年26,916,993名、Td/TdapおよびRZVは、2019年18,752,789名、2020年18,701,076名であり、2020年1月5日～3月14日のメディケア受給者における週間接種率は2019年の同じ週と比べ上回っていたが、3月13日の国家非常事態宣言後では、接種率は急激に減少した[－25％(PPSV23)～－62％(RZV)]。低下が最大であったのは、PCV13、PPSV23、Td/Tdapでは4月5日～11日の週(それぞれ－88％、－80％、－70％)、RZVでは4月12日～18日の週(－89％)であり、その後は徐々に回復し、観察最終週(7月12日～18日)の週間ワクチン接種率はPPSV23にて2019年より8％高かったが、他のワクチンではいずれも低いままであった。人種/民族別での接種率の変動パターンは全体と同様であったが、最終週にて白人ではPPSV23接種率が2019年より10％高く、PCV13およびTd/Tdapはそれぞれ－42％、－23％ともっとも低下が小さかった。接種率の変動は、ヒスパニック/ラテン系が－62％(PCV13)～－22％(PPSV23)、アジア人が－57％(PCV13)～－9％(PPSV23)、黒人が－44％(PCV13)～－2％(PPSV23)、その他人種/民族が－44％(RZV)～＋3％(PPSV23)であった。以上、成人の予防接種率はCOVID-19非常事態宣言により前年から最大70～89％下回ったが、徐々に回復していた。ワクチン供給者は成人患者に対し、定期予防接種を継続することの重要性を強調し、高齢者をワクチンで予防可能な疾患から守るため、COVID-19パンデミック進行中においても安全なワクチン提供を行うべきである。
