Trends in Use of Telehealth Among Health Centers During the COVID-19 Pandemic — United States, June 26–November 6, 2020
COVID-19パンデミック中の保健センターにおける遠隔医療利用の傾向 ― アメリカ、2020年6月26日~11月6日
遠隔医療は治療へのアクセスを促進し、SARS-CoV-2[新型コロナウイルス感染症(COVID-19)の原因ウイルス]感染リスクの軽減、十分でない医療用品の節約、さらに治療継続性をサポートしながら医療のキャパシティと施設への負担を軽減することから、Health Resources and Services Administration(HRSA)が資金を提供している保健センターにてCOVID-19パンデミック中に遠隔医療サービスが拡大されている。2020年4月、HRSAは保健センターの検査能力、手術、患者、職員に及ぼすCOVID-19の影響を追跡するため、週1回、Health Center COVID-19 Surveyを開始した。CDCとHRSAは1,382の保健センターのうち、連続した20週間(2020年6月26日が週末の週~2020年11月6日が週末の週)に回答した245の保健センターのデータを分析した。この期間、遠隔医療の受診率は6月26日の週の35.8%から11月6日の週には26.9%へ25%低下し、平均30.2%であった。地域別では北東部(50施設)が37.7%、中西部(41施設)が28.4%、南部(77施設)が20.4%、西部(72施設)が36.3%、プエルトリコ(5施設)が31.3%であり、前半10週間(6月26日の週~8月28日の週)の変化は北東部が-13.3%、中西部が-7.3%、南部が-1.1%、西部が-4.7%、プエルトリコが-3.0%、後半10週間(9月4日の週~11月6日の週)では北東部が+0.1%、中西部が-3.3%、南部が-2.7%、西部が-3.0%、プエルトリコが-1.0%であった。また、都市部(154施設)における全期間での利用は35.2%であり、前半10週間は-7.6%、後半10週間は-3.0%の変化、地方(91施設)における全期間での利用は21.7%であり、前半10週間は-2.6%、後半10週間は-1.1%の変化であった。以上、2020年6月26日の週~11月6日の間にアメリカ全体にて保健センター受診の30.2%が遠隔医療にて行われ、新規COVID-19症例数の減少に伴い、遠隔医療による受診は減少したが、症例数が増加した時も水平状態を維持していた。COVID-19パンデミックは継続しており、遠隔医療の拡大を維持することは医療アクセスを提供するために依然として重要である。
