Vital Signs: Prevalence of Multiple Forms of Violence and Increased Health Risk Behaviors and Conditions Among Youths — United States, 2019
バイタルサイン:若者における多様な形の暴力の蔓延と健康上のリスクがある行動および状態の増加 ― アメリカ、2019年
高学年齢の若者が経験する暴力は重大な公衆衛生的問題である。アメリカでは14~18歳の死因の第3位は殺人であり、毎日約360名の若者が非致死性の肉体的暴力により救急外来で治療されている。さまざまな種類の暴力[喧嘩、デート中の暴力、いじめなど]を経験する若者も多い。小児期における暴力などの有害な経験は、注意、決断、学習、感情調節に関連する脳の化学的および物理的発達に負の影響を与える。今回、2019年のYouth Risk Behavior Survey のデータを使用し、アメリカ高校生(9~12年生)における調査前12カ月の暴力(身体的けんか/武器による脅迫、デート中の身体的暴力、性的暴力、いじめ)の自己申告による経験について判断した。回答率は80.3%、全体のサンプル人数は13,677名であった。調査前12カ月以内に1種類、2種類、3種類以上の暴力を経験した生徒はそれぞれ28.7%、10.8%、4.8%であり、44.3%が1種類以上、15.6%が2種類以上の暴力を経験していた。3種類以上の暴力を経験したのは、女子生徒(6.1%)にて男子生徒(3.4%)よりも多く、ゲイ/レズビアン/バイセクシュアル(9.6%)、性的指向不明(7.4%)の生徒より、異性愛者(3.8%)の生徒よりも多かった。暴力の経験は検討したすべての健康上のリスク行動および状態と有意に関連しており、3種類以上の暴力を経験した生徒では健康上のリスクが高い生徒が多く、安全性の懸念から欠席(34.1%)、成績不良(33.8%)、学校への武器持ち込み(13.4%)、銃の持ち込み(18.9%)、物質使用(処方鎮痛薬の誤使用:30.1%~電子タバコ製品の使用:65.4%)、危険な性行動(複数と性交:21.0%~避妊具不使用:63.4%)、体重過多/肥満(39.1%)、自殺念慮/自殺行為(71.4%)、悲しみや絶望を感じる(78.4%)などが多く認められた。暴力を経験し生徒における健康上のリスクは過体重/肥満を除く15項目にて暴力を経験していない生徒に比べ有意に高く、2種類の暴力を経験した生徒では避妊具不使用を除く15項目にて暴力を経験していない生徒に比べて有意に高かった。さらに3種類以上の暴力を経験した学生では16項目すべてで暴力を経験していない生徒に比べて有意に高かった。以上、多くの若者が複数の種類の暴力を経験しており、生涯にわたる健康上への影響の可能性がある。暴力は個人、家族、環境でのリスクに対処した確かなアプローチで予防できる。
