Sexual Orientation Disparities in Risk Factors for Adverse COVID-19–Related Outcomes, by Race/Ethnicity — Behavioral Risk Factor Surveillance System, United States, 2017–2019

現在のCOVID-19サーベイランスシステムでは性的指向に関する情報は記録していない。性的マイノリティ成人におけるCOVID-19リスクに関する知識のギャップを埋めるため、重症の新型コロナウイルス感染症(COVID-19)関連疾患に関連する強い、または複合的なエビデンスのある基礎疾患の有病率について、性的マイノリティ成人と異性愛者との差について、2017～2019年のBehavioral Risk Factor Surveillance Systemデータを用いて評価した。性的指向に関する質問を実施した州の回答率中央値は、2017年が42.3％、2018年が48.5％、2019年が46.4％であり、今回の分析では「ゲイ」、「ゲイまたはレズビアン」、「バイセクシュアル」のいずれかを選択した回答者を性的マイノリティとして分類した。全体の回答者643,956名のうち、性的マイノリティは24,582名(4.7％)、異性愛者は619,374名(95.3％)であった。COVID-19の重症化を増加させるリスクが高い健康状態には、癌、慢性腎臓病、慢性閉塞性肺疾患(COPD)、心疾患、肥満、妊娠、鎌状赤血球症、喫煙、2型糖尿病があり、さらに喘息、脳血管疾患、嚢胞性線維症、高血圧、免疫不全状態、神経学的状態(認知症など)、肝臓病、過体重、肺線維症、サラセミア、1型糖尿病はCOVID-19重症化のリスクが高い可能性がある。これらの健康状態のうち、今回の調査では喘息(現在および既往)、がん(非メラノーマ皮膚がんを除く)、COPD、心疾患(心筋梗塞、狭心症、冠状動脈疾患)、糖尿病、高血圧、腎臓病、肥満(現在)、喫煙(現在)、脳卒中が含まれ、高血圧は2017年、2019年のみ質問されていた。すべての人種/民族において、性的マイノリティでは異性愛者に比べ、喘息(現在および既往)、がん、心疾患、COPD、高血圧、腎臓病、肥満、喫煙、脳卒中の調整後の割合(aPR)が高く、非ヒスパニック系黒人では喘息、COPD、喫煙が、非ヒスパニック系白人では喘息、がん、COPD、糖尿病、高血圧、腎臓病、肥満、喫煙、脳卒中が、ヒスパニック系では喘息、がん、COPD、喫煙、脳卒中、非ヒスパニック系その他人種では喘息、がん、心疾患、COPD、肥満、喫煙のaPRが高かった。以上、アメリカでは性的マイノリティのCOVID-19重症化に関連する健康状態の割合が高かった。COVID-19サーベイランスシステムに性的指向や性別のデータを含めることにより、感染や有害転帰における格差に関する知識が改善される可能性がある。
