Allergic Reactions Including Anaphylaxis After Receipt of the First Dose of Moderna COVID-19 Vaccine — United States, December 21, 2020–January 10, 2021

Moderna社製COVID-19ワクチンの初回接種後におけるアナフィラキシーを含むアレルギー反応 ― アメリカ、2020年12月21日～2021年1月10日

2021年1月20日時点で、アメリカでは新型コロナウイルス感染症(COVID-19)の症例は計24,135,690例、関連する死亡が400,306例、報告されている。2020年12月18日、FDAは、COVID-19予防として1カ月間隔で2回接種するModerna社製COVID-19ワクチンの緊急使用許可(EUA)を発表した。2020年12月19日、Advisory Committee on Immunization Practicesは、Moderna社製COVID-19ワクチン使用に関する暫定勧告を発表した。2021年1月10日の時点で、アメリカでは4,041,396例[女性2,465,411例(61％)、男性1,450,966例(36％)、性別記録なし125,019例(3％)]にModerna社製COVID-19ワクチンの初回接種がされたとCDCに報告された。今回、アメリカで2020年12月21日～2021年1月10日にModerna社製COVID-19ワクチンの初回接種後におけるアナフィラキシーおよび非アナフィラキシーのアレルギー反応を含むアレルギー反応の報告症例の臨床的および疫学的特徴の要約を報告する。同期間におけるModerna社製COVID-19ワクチン初回接種後の有害事象は1,266件(0.03％)がVaccine Adverse Event Reporting Systemに報告された。このうち108例の報告がアナフィラキシーを含む重度アレルギー反応の可能性がある症例として確認され、さらに調査された。アナフィラキシーは生命を脅かすアレルギー反応であり、ワクチン接種後の発現はまれではあるが、通常は数分から数時間以内に発現する。これらの症例報告のうち10例(全例女性、年齢中央値47歳、31～63歳)がアナフィラキシーと判断された(Moderna社製COVID-19ワクチン初回接種100万回あたりアナフィラキシー発現例の割合は2.5例）。このうち9例にアレルギーまたはアレルギー反応の既往があり[薬物(6例)、造影剤(2例)、食品(1例)]、5例にアナフィラキシーの既往があったが、ワクチン接種関連はなかった。ワクチン接種から症状発現までの期間中央値は7.5分(1～45分)であった。追跡調査の情報が明らかな8例では、全員が回復、または退院していた。アナフィラキシーと判断されなかった残りの報告症例のうち、47例が非アナフィラキシーのアレルギー反応、47例が非アレルギー性有害事象と考えられた。4例では、アナフィラキシーの可能性を評価するための十分な情報が得られなかった。CDCは、現在アメリカで認可されているmRNA COVID-19ワクチンの使用のための最新の暫定的な臨床的検討事項と、アナフィラキシー可能性に対しての治療準備に関する暫定的な検討事項について発表している。
