Evaluation of Abbott BinaxNOW Rapid Antigen Test for SARS-CoV-2 Infection at Two Community-Based Testing Sites — Pima County, Arizona, November 3–17, 2020
2つの地域ベースの検査会場におけるSARS-CoV-2感染症のAbbott BinaxNOW迅速抗原検査の評価 ― アリゾナ州ピマ郡、2020年11月3日~17日
Abbott社のBinaxNOW COVID-19 Ag Card(BinaxNOW)などの迅速抗原検査は、感度の高い核酸増幅検査に比べて結果が出るのが早く(約15~30分)、費用も低い。迅速抗原検査は症状のある人を対象とし、FDAの緊急使用許可を受けているが、感染者を迅速に特定して隔離するための拡大スクリーニング検査をするには、無症状患者対象の検査能力に関するデータが不足している。今回、BinaxNOW迅速抗原検査の性能を評価するため、アリゾナ州ピマ郡の2カ所の検査会場(A会場11月3日~17日、B会場11月8日~16日)にて、10歳以上の人3,419名から採取した上気道検体(3,419対)の分析をリアルタイムRT-PCR検査とともに行った。上咽頭検体を採取後すぐに会場にBinaxNOW抗原検査を行い、リアルタイムRT-PCR検査は24~48時間後に実施した。検査参加者の年齢中央値は41歳(10~95歳)、10~17歳が236名(6.9%)、18~49歳が1,885名(55.1%)、50~64歳が743名(21.7%)、65歳以上が555名であった(16.2%)。男性1,290名(37.7%)、女性1,681名(49.2%)、不明448名(13.1%)であり、約1/3がヒスパニック/ラテン系(1,075名、31.4%)、約3/4が白人(2,567名、75.1%)であった。検査の時点で827名(24.2%)に新型コロナウイルス感染症(COVID-19)に一致する徴候または症状があり、2.592名(75.8%)は無症状であった。症状のあった人のうち、113名(13.7%)がBinaxNOW抗原検査陽性、176名(21.3%)がRT-PCR検査陽性であり、無症状の人ではそれぞれの検査で48名(1.9%)、123名(4.7%)が陽性であった。症状のあった人では、BinaxNOW抗原検査の感度は64.2%、特異度は100%、陽性適中率(PPV)は100%、陰性適中率(NPV)は91.2%であり、症状発現から7日以内の人では感度は71.1%、特異度は100%、PPVは100%、NPVは92.7%であった。無症状の人では感度は35.8%、特異度は99.8%、PPVは91.7%、NPVは96.9%であった。RT-PCR検査を基準とした場合、BinaxNOW抗原検査では偽陽性が4検体にて認められ、すべて無症状の人の検体であった。リアルタイムRT-PCR陽性であった299検体のうち、142検体(47.5%)がBinaxNOW抗原検査で偽陰性であった(症状あり63名、無症状79名)。いずれかの検査で陽性であった274検体のうち96検体[両検査とも陽性:85/147検体(57.8%)、BinaxNOW抗原検査偽陰性:11/124検体(8.9%)]からウイルスが採取され、CDC 2019-nCoV Real-Time RT-PCR Diagnostic Panelにて分析した結果、サイクル閾値(Ct値)はBinaxNOW偽陰性検体にて有意に高く[33.9 vs. 22.0(症状あり)、、33.9 vs 22.5(無症状)]、ウイルス培養陽性検体のCt値の中央値は22.1であり、培養検査陰性検体(32.8)に比べ有意に低かった。以上、BinaxNOW抗原検査の感度は、RT-PCR検査に比べ無症状の人の検体では感度が低かったが特異度は高く、迅速抗原検査は感染者の早期に派遣し、隔離するために有用である可能性が示唆された。
