COVID-19 Case Investigation and Contact Tracing Efforts from Health Departments — United States, June 25–July 24, 2020
保健局によるCOVID-19症例調査および接触者追跡の取り組み ― アメリカ、2020年6月25日~7月24日
症例調査および接触者追跡は新型コロナウイルス感染症(COVID-19)の原因ウイルスであるSARS-CoV-2を含む病原体の感染防止に用いられる主要な公衆衛生的ツールであり、とくに適時性が有効性に重要である。2020年5月、CDCは64の州、地方、管轄領土の保健局にCOVID-19対応活動の支援として資金を提供しており、モニタリング行程の一部として62の保健局から6月25日~7月24日における症例調査および接触者追跡の指標が報告された[1:調査員1名あたりの調査症例数、追跡接触者数および人員配置モデル、2:症例調査の適時性(24時間以内の調査)、3:接触者追跡の適時性(24時間以内に感染の可能性を通知)、4:接触者追跡率(接触者のうち診察に至った人の割合)]。報告のあった62保健局のうち、症例がいなかった4カ所(すべて太平洋諸島)およびデータの提出が一部のみであった2カ所を除いた56保健局のデータを分析した。この期間の調査症例数は中央値8,306例(22~280,033例)であり、調査員1名あたりの調査症例数は中央値31例(1~196例)、24時間以内に優先的に調査された症例数は中央値57例(1~100例)であった。また、追跡接触者数は中央値7,498名(124~95,755名)、調査員1名あたりの追跡接触者数は中央値29名(0.5~200名)、症例判明から24時間以内に通知された接触者数は55名(4~100名)であった。49保健局における調査員1名あたりの症例数と症例調査の適時性は逆相関を示し、調査員1名あたりの症例数が少ない保健局では24時間以内に問診を完了した症例の割合が高く、90%以上の症例に24時間以内に診察を完了した4つの保健局では調査員1名あたりの症例数が30例未満であり、調査員1名あたりの症例数が130例を超える4つの保健局では24時間以内に診察を完了した症例は30%未満であった。以上、人員の増員および能力の向上、地域活動の改善により、適時的な症例追跡調査、より多くの接触者の確認が強化される可能性がある。
