Rates of COVID-19 Among Residents and Staff Members in Nursing Homes — United States, May 25–November 22, 2020
介護施設の居住者と職員におけるCOVID-19の割合 ― アメリカ、2020年5月25日~11月22日
新型コロナウイルス感染症(COVID-19)パンデミックの初期に、介護施設はCOVID-19アウトブレイクのリスクが高い集合環境として特定された。また、介護施設の居住者は高齢および特定の基礎疾患と関連する重症化転帰の観点から、COVID-19の原因ウイルスであるSARS-CoV-2の感染に関連した疾病率および死亡率のリスクが一般集団よりも高い。CDCのNational Healthcare Safety Network(NHSN)は、2020年4月26日に、全国の施設別でのCOVID-19介護施設サーベイランスを開始した。Centers for Medicare & Medicaid Servicesにより発行された連邦指令により、介護施設は登録と2020年5月25日までに居住者と職員におけるCOVID-19症例の登録および定期報告を開始することを求められた。今回、2020年5月25日~11月22日に報告されたNHSNの介護施設のCOVID-19データを使用し、介護施設の居住者および職員におけるCOVID-19の割合を算出し、対応するアメリカ保健福祉省の(HHS)の地域別に、周辺地域の割合と比較した。同期間に介護施設はNHSNに572,135例を報告し、そのうち296,762例(51.8%)が居住者で、275,373例(48.2%)が職員での発症であった。居住者における1,000人-週あたりの罹患率は、6月から7月にかけて増加し、11.5例に達し[7月26日の週(毎週のデータの報告日における総病床使用数で計算)]8月から9月にかけて減少し、6.3例となったが(9月13日の週)、11月には再び増加し、1,000人-週あたり23.2例に達した(11月22日の週)。職員における1,000人-週あたりの罹患率も6月から7月にかけて増加し、10.9例に達し(7月26日の週)、8月から9月にかけて減少し、6.3例となったが(9月13日の週、6.3例)、11月には増加し、21.3例に達した(11月22日の週)。各HHS地域における周辺地域住民におけるCOVID-19の割合も同様の傾向であった。地域のCOVID-19罹患率増加は、介護施設における罹患率増加にも関連している可能性があり、介護施設の感染緩和策には、地域のSARS-CoV-2感染を監視し、施設内の高リスク感染を最小限にするために包括的な計画を含める必要がある。
