Time from Start of Quarantine to SARS-CoV-2 Positive Test Among Quarantined College and University Athletes — 17 States, June–October 2020

隔離された大学生アスリートにおける隔離開始からSARS-CoV-2陽性検査までの期間 ― 17州、2020年6～10月

単科大学および総合大学の体育プログラムと地域のアスレティックカンファレンスはスポーツを安全に再開するため、新型コロナウイルス感染症(COVID-19)の原因ウイルスであるSARS-CoV-2感染の緩和策の計画を立てた。緩和策には、物理的距離、マスク常時着用、トレーニング時における屋外活動の最大化、定期検査、COVID-19感染者の10日間隔離、確定診断されたCOVID-19感染者の濃厚接触者として確認されたアスリートの14日間隔離などが含まれた。地域のアスレティックカンファレンスはNational Collegiate Athletic Association(NCAA)指針に基づいて検査および隔離の方針をたてた。隔離のコンプライアンスを改善し、隔離アドヒアランスに関する個人的負担および経済的負担を軽減するため、いくつかの国では隔離期間を14日間から検査をした場合には5日間、検査なしでは10日間に短縮している。今回CDCは　NCAAカンファレンスの担当者と連携し、調査に参加した大学が収集したCOVID-19に曝露して隔離された大学生アスリートのデータをレトロスペクティブに分析した。24大学から2,257名の隔離されたアスリートに関するデータが提供され、基準を満たさない427名のデータを除外し、1,830名を分析対象とした。運動の種類はフットボール(846名、46.2％)、陸上/クロスカントリー(190名、10.4％)、サッカー(121名、6.6％)、バスケットボール(116名、6.3％)、バレーボール(107名、5.9％)、水泳/ダイビング(78名、4.3％)、野球(72名、3.9％)などであった。COVID-19曝露の機会は、集会(745名、40.7％)、ルームメイト(580名、31.7％)、競技活動(232名、12.7％)であった。アスリートが隔離中、総計3,345回のRT-PCR検査が行われた(1人あたり平均1.8回、1～14回)。458名(25.0％)の検査結果が陽性であり、そのうち137名(29.9％)には症状がなかった。症状のあった386名では、321名(83.2％)が検査結果陽性であった。3つの大学からは隔離中の検査結果が陽性であったアスリートのみのデータが提出された(193名)。そのうち基準を満たさない31名を除外した162名を加え、計620名の隔離中のSARS-CoV-2検査結果が陽性であったアスリートのデータを対象にtime-to-event分析を行った。曝露日の明らかな436名(73.4％)の隔離開始日は曝露後平均1.1日(0～11日)であった。検査用の検体採取前に症状があった選手は302名(48.7％)、症状を認めてから検体採取までの平均期間は1.1日(0～12日)であった。また、検査結果陽性の判明は303名(48.9％)が隔離2日目までに、453名(73.1％)は隔離5日目までに得られており、隔離開始から陽性判明までの平均日数は3.8日(0～14日)であった。5日目時点で検の査結果が陰性であったアスリートのうち、5日目、7日目、10日目以降に陽性となったのはそれぞれ26.9％、14.2％、4.7％であり、11～14日に陽性となった29名のうち、26名(89.7％)はそれまでの隔離期間中、検査を受けていなかった。以上、COVID-19に曝露した大学生アスリートのうち、1/4が隔離期間中に陽性となり、5日目まで陰性であった選手の陽性率は5日目の27％から10日目には5％未満に低下し、若い健康なアスリートでは隔離10日目以降に陽性となる確率は低いことが示された。
