Telehealth Practice Among Health Centers During the COVID-19 Pandemic — United States, July 11–17, 2020

COVID-19パンデミック中の医療センターにおける遠隔医療の実施 ― アメリカ、2020年7月11日～17日

アメリカでは新型コロナウイルス感染症(COVID-19)パンデミックの初期において、対面による外来診療は60％減少したのと同時に遠隔医療による診療が増加し、一部の地域では総診療の最大30％を占めた。2020年3月、Centers for Medicare & Medical ServicesはCOVID-19 Public Health Emergency中における遠隔医療の提供に関する規定およびガイダンスの改定に関する最新情報を発表した。CDCおよびHealth Resources and Services Administration(HRSA)は2020年7月11日～17日の週にHRSAが資金提供している1,009施設から毎週報告されるCOVID-19調査データから、遠隔医療サービスの使用について分析した。遠隔医療は963施設(95.4％)にて実施され、513施設(50.8％)が0～30％、450施設(44.6％)が30％以上の割合であり、46施設(4.6％)では実施されていなかった。各地域の医療センターのうち、地方の医療センターが占める割合は北東部が30.5％、中西部が40.8％、南部が47.0％、西部が40.4％、アメリカ領/自由連合盟約国が65.4％であり、遠隔医療の実施率が30％以上であった医療機関の割合は、北東部が56.1％、中西部が36.9％、南部が31.1％、西部が58.9％、アメリカ領/自由連合盟約国が57.7％であった。南部と比較した遠隔診療の普及率は北東部が1.81、中西部が1.19、西部が1.90、アメリカ領/自由連合盟約国が1.86であり、全体で都市部の医療センターの55.1％、地方の医療センターの29.9％にて遠隔慰労の実施が30％以上であった。職員が足りていた医療機関では職員が5％不足していた医療機関に比べ、遠隔医療の実施率が30％を超える施設が22％多く、職員が10％以上不足していた医療機関に比べ63％多かった。また、遠隔医療受診率と個人保護具(PPE)不足または2019年からの受診数の変化との間に関連性は認めなかった。遠隔医療は治療へのアクセスを促進および拡大するたに有望な方法であり、費用対効果が高く、公衆衛生上の緩和政策を促進し、COVID-19、その他の呼吸器疾患の感染拡大の防止にも有効である。
