Trends in U.S. Emergency Department Visits Related to Suspected or Confirmed Child Abuse and Neglect Among Children and Adolescents Aged <18 Years Before and During the COVID-19 Pandemic — United States, January 2019–September 2020

新型コロナウイルス感染症(COVID-19)パンデミックによるストレス増大、学級閉鎖、所得喪失、社会的孤立で児童虐待やネグレクトのリスクが増大している。CDCは、2019年1月6日～2020年9月6日のNational Syndromic Surveillance Programデータを使用して、児童虐待とネグレクトに関連する救急科(ED)の週ごとの受診数を集計し、全体および年齢層別(0～4歳、5～11歳、12～17歳)でのED受診10万件あたりに占める割合、入院となった割合を算出した。第11週(2020年3月15日～3月22日)では、調査したすべての年齢層で、児童虐待とネグレクトに関連する総ED受診数は減少し始め、2019年の同期間を下回り、3月13日の国家非常事態の宣言と合致していた。同時に、ED受診10万件あたりに占めるこれらの受診の割合は、すべての年齢層で2019年と比べて増加し始めた。18歳未満の小児と青年での児童虐待とネグレクトに関連するED受診は、第13週(2020年3月29日～4月4日)に最低となった。この初期のパンデミックでの最低値に続く4週間(3月29日～4月25日)では、18歳未満の小児と青年の児童虐待とネグレクトに関連するED受診数は、2019年の同期間(3月31日～4月27日)の受診数よりも平均で53％低下した。児童虐待とネグレクトに関連するEDの受診数は、すべての年齢層の2019年の同期間の受診と比較して、2020年のこの期間に少なく、5～11歳の小児による受診数で最大の比例減少を示した。2020年では、児童虐待とネグレクトに関連するED受診の総数は減少したにもかかわらず、入院につながるこれらのED受診数は減少しなかった。変わらない入院数と全体的なED受診数の減少の結果、入院にいたった児童虐待とネグレクトに関連するED受診の割合は、18歳未満の小児と青年の間で2019年の2.1％から2020年には3.2％に有意に増加した。児童虐待やネグレクトに関連するED受診の割合の増加は、ED受診の総数の減少に関連している可能性があるが、これらの調査結果は、パンデミック中に医療を求めるパターンが変化していたことも示唆している。児童虐待とネグレクトによる入院は2020年には減少せず、これは、パンデミック中にED受診が減少したにもかかわらず、傷害の重症度が低下しなかったことを示唆している。児童虐待は防止でき、世帯の経済的支援の強化や家族に優しい労働政策の作成をはじめとする戦略の実施は、困難な時期のストレスを軽減し、安全で安定した養育関係や環境で発育する子供たちの機会を増やすことができる。
