The Advisory Committee on Immunization Practices’ Ethical Principles for Allocating Initial Supplies of COVID-19 Vaccine — United States, 2020

COVID-19ワクチンの初期供給の割り当てに関するAdvisory Committee on Immunization Practicesの倫理原則 ― アメリカ、2020年

新型コロナウイルス感染症(COVID-19)の原因ウイルスであるSARS-CoV-2の蔓延と、それに関連する健康と社会への影響を減らすためには、COVID-19ワクチンが不可欠である。アメリカ政府は、アメリカの全国民に対して安全で有効なCOVID-19ワクチンの製造と提供に取り組んでいる。Advisory Committee on Immunization Practices(ACIP)は、FDAにより緊急使用許可または認可のために承認された各COVID-19ワクチンの使用に関する推奨事項を作成するため、アプローチの概要を説明している。ACIP推奨の課程には、ワクチンの安全性および有効性を評価するための明確で透明性のあるエビデンスに基づく方法ともに、実施を含む他の要因の検討が含まれている。おそらくワクチンの初期供給には制限があるため、ACIPは、どの層が最も早いワクチン割り当てを受けるべきかについても推奨する予定である。ACIPのCOVID-19ワクチンワーキンググループおよび倫理と健康の公平性に関する専門知識を持つコンサルタントは、外部の専門家委員会の報告書と公表された文献を検討し、COVID-19ワクチンの割り当て決定に関連する倫理的問題を審議した。今回の報告では、科学的データと実施の実現可能性に加えて、供給が制限されているときにACIPがCOVID-19ワクチンの割り当てに関する推奨事項を策定するのに役立つ4つの倫理原則を説明した[1)利益の最大化と損失の最小化(個人と集団全体の両方に対するワクチン接種の利点にはSARS-CoV-2感染とCOVID-19に関連する罹患率と死亡率の減少が含まれ、これにより、緊迫した医療能力と施設への負担の軽減、COVID-19対応に不可欠なサービスの温存、全体的な社会的機能の維持が図られる)、2)公平さの推進(すべての人が最大限の健康と幸福をできるかぎり享受するための平等な機会を保護し、前進させる責務)、3)健康の不平等の緩和(健康の公平性はすべての人に自分の完全な健康の可能性を達成する機会があり、社会的地位または他の社会的に決定された状況のために、不利益を被らないときに達成)、4)透明性の促進(透明性は意思決定プロセスに関連しており、ワクチンプログラムの計画と実施中に公衆の信頼を築き維持するために不可欠)]。これらの原則は、州、部族地帯、地方、領土内の公衆衛生当局がACIP推奨に基づいて、地域内のワクチン実施戦略を開発する時にも役立つと考えられる。
