Implementation of Hospital Practices Supportive of Breastfeeding in the Context of COVID-19 — United States, July 15–August 20, 2020

COVID-19の状況下で母乳育児を支援する病院診療の実施 ― アメリカ、2020年7月15日～8月20日

母乳育児には乳児と母親の双方に健康上の利点があり、多くの保健期間および医療機関から推奨されている。出産のための入院は、母乳育児を確立するために重要な時期である。しかし、一部の病院診療、特に母親と新生児の接触に関連する診療では、新型コロナウイルス感染症(COVID-19)の原因ウイルスであるSARS-CoV-2が母親から新生児へ感染する可能性の懸念が生じている。CDCは、現在の診療と入院中の母乳育児支援を評価するために、2018年のMaternity Practices in Infant Nutrition and Care調査を完遂した1,344の病院を対象にCOVID-19調査を実施した(2020年7月15日～8月20日)。情報が明らかな1,343の病院のうち、724の病院(53.9％)にてCOVID-19と確定診断された母親から出生した乳児1～19例に対処し、20例以上に対処したのは152施設(11.4％)、457施設(34.0％)では対処例はなかった。年間出生数は、病院の約半数で1,000人未満、42.0％が1,000～4,999人、2.8％が5,000人以上であり、病院の大多数(78.8％)は非営利であった。COVID-19が疑われる、または確定診断された母親に対し、新生児の皮膚と皮膚との接触について、14.0％の病院では控えさせ、6.5％では禁止し、母子同室については37.8％の病院が控えさせ、5.3％が禁止した。直接的な母乳育児については、20.1％の病院では控えさせたが、母親が希望する場合は許可し、12.7％では直接的な母乳育児を支援せず、健康人からの搾母乳の授乳を奨励した。COVID-19パンデミックのため、17.9％の病院で対面での授乳支援が減少し、72.9％の病院が出産後48時間未満で母親と新生児を退院させた。病院が実施した一部の感染予防対策の実施は、母乳育児を支援するためのエビデンスに基づくケアと矛盾していた。新生児から引き離されている、または直接授乳していない母親に対しては、退院後の母乳育児の支援がさらに必要になる可能性がある。さらに、アメリカ小児科学会は、出生から48時間より前に退院した新生児は、小児医療ケア従事者による迅速な経過観察を受けることを推奨している。
