深在性真菌症に対する国民の意識 ― アメリカ、2019年
真菌症は軽微な皮膚やおよび粘膜の感染症から生命を脅かす播種性疾患まで多岐にわたり、直接医療費の年間推定額は72億ドルを超える。これらの疾患は過小診断される傾向にあり、診断および治療の遅れを改善するには医療関係者および一般市民の認識の向上が重要である。今回、オンラインのPorter Novelli’s Fall 2019 ConsumerStyles Surveyを利用して、18歳以上の4,677名に真菌感染症[アスペルギルス症、カンジダ感染(カンジダ症)、コクシジオイデス症(渓谷熱)、クリプトコッカス症、ブラストミセス症、ヒストプラズマ症]について聞いたことがあるかを質問した。回答率は77.5%で3,624名(男性1,756名、女1,868名、平均47歳、18~94歳)が調査を完了した。全体で31.1%がいずれかの真菌性疾患について聞いたことがあると回答し、認知度はブラストミセス症(4.1%)がもっとも低く、次いでアスペルギルス症(5.1%)、ヒストプラズマ症(7.5%)、コクシジオイデス症(7.6%)、クリプトコッカス症(9.0%)、カンジダ症(24.6%)の順で低かった。1つの真菌性疾患を知っている人では他の真菌性疾患についても認知度が高い傾向であった。女性、教育レベルの高さ、処方薬の多さは、いずれの疾患でも認知度と関連性があり、特に女性のカンジダ症の認知度は男性に比べて3倍以上高かった。処方薬数の多さは認知度の6~11%上昇と関連し、非白人または多民族の回答者では白人に比べ、カンジダ症、コクシジオイデス症の認知度が低かった。居住地域別では、西部が中西部に比べ、コクシジオイデス症の認知度の高さと有意に関連し、ブラストミセス症の認知度は中西部に比べ、北東部および南部にて低かった。真菌性疾患に対する国民の認識に関する調査は今回が初めてであり、今後の認識の違いについて評価する研究のベースラインとなるものである。また、真菌性疾患に対する国民の認知度を高めるため、継続的な戦略が必要であると思われる。
