Facility-Wide Testing for SARS-CoV-2 in Nursing Homes — Seven U.S. Jurisdictions, March–June 2020

ナーシングホームにおけるSARS-CoV-2の全施設検査 ― 7つのアメリカ管轄地域、2020年3月～6月

新型コロナウイルス感染症(COVID-19)の原因ウイルスであるSARS-CoV-2の未検出感染はナーシングホームにおける感染の一因となり、入居者の死亡率が高い大きなアウトブレイクを引き起こす。入居者および医療ケア従事者(HCP)を対象とした全施設検査は、無症候性および発症前の感染の特定と感染予防対策の介入の手助けとなる。2020年3月24日～6月14日、7つの州および地域の保健局は288施設にて全施設検査を実施した。5地域(アーカンソー州、ミシガン州デトロイト、ニューメキシコ州、ユタ州、バーモント州)では全施設検査の対象施設を絞り、93施設、13,443名に検査を実施し、79％の施設にて新規COVID-19症例が1施設あたり中央値6例確認された。陽性例は1,619名(12％)であり、88施設(95％)では検査前にCOVID-19患者が確認されていた。入居者とHCPにおける陽性例の相関性については62施設のデータが分析され、入居者3例あたりHCP 1.3例が陽性であり、入居者にSARS-CoV-2検査陽性者がいた45施設(73％)では、HCPの検査結果陽性率は平均5.2％(0～26％)であった。2地域(ノースダコタ州、サウスダコタ州)では州内の全ナーシングホームを全施設検査の対象とし、195施設、37,465名に検査を実施した。そのうち70施設(36％)では今回の検査前にSARS-CoV-2陽性例が確認されており、検査を受けた14,488名中331名(2％)が検査結果陽性であった。今回の検査前に陽性例が確認されていなかった125施設(64％)では、検査を受けた22,977名のうち95名(0.4％)が検査結果陽性であった。ナーシングホームにおける症例が確認された後の迅速な全施設検査の実施は、入居者およびHCPにおける感染対策に役立つかもしれない。症例が確認されていないナーシングホームでも適切な全施設検査の戦略は必要である。
