Initial and Repeated Point Prevalence Surveys to Inform SARS-CoV-2 Infection Prevention in 26 Skilled Nursing Facilities — Detroit, Michigan, March–May 2020

高度看護施設26カ所におけるSARS-CoV-2感染予防について通知する初回および複数回の点有病率調査 ―ミシガン州デトロイト、2020年3月～5月

高度看護施設(SNF)は新型コロナウイルス感染症(COVID-19)パンデミックの中心であり、COVID-19の原因ウイルスであるSARV-CoV-2による無症候性感染がSNF入所者と医療従事者において報告されている。一方、無症候性感染を特定するために複数回実施される点有病率調査(症状に関係なく医療施設のすべての入所者と医療従事者を対象とした連続した検査)が利用されており、SNFアウトブレイク時のSARS-CoV-2感染が減少している。2020年3月、デトロイト保健局と地域の病院は、SNF入所者におけるCOVID-19診断、入院、関連する死亡の急激な増加に気づいた。デトロイト保健局は、地方自治体、学術機関、医療ケアシステムパートナー、CDCフィールドチームと協同し、デトロイト地区すべてのSNFでSARS-CoV-2検査を迅速に拡大し、感染対策(IPC)活動を実施した。3月7日～5月8日、デトロイトにおける26カ所のSNFの入居者2,773名のうち、1,207例のCOVID-19症例が検査で確認された。確認されたのは調査前(3月7日～4月7日)、2回の点有病率調査期間(4月8日～25日、4月30日～5月8日)であり、全体の発病率は44％であった。COVID-19患者1,207例の年齢中央値は72歳であった。初回の陽性結果が出てから21日以内に446例(37％)が入院し、入院患者の233例を含む287例(24％)が死亡した。両方の調査期間に参加した12施設では、検査結果陽性の割合が35％から18％に減少した。SNFで複数回実施した点有病率調査により、無症候性COVID-19症例が特定され、感染の減少を目的としたコホーティングの情報とIPC実施、および高レベルのSARS-CoV-2感染が発生している施設の保健部門のリソースの優先順位付けがなされた。SARS-CoV-2検査の供給増加により、複数回の点有病率調査とIPC支援の強化および拡張は、SNFにおけるCOVID-19アウトブレイクの阻止および予防のための標準ツールになるであろう。
