Provision of Pediatric Immunization Services During the COVID-19 Pandemic: an Assessment of Capacity Among Pediatric Immunization Providers Participating in the Vaccines for Children Program — United States, May 2020

最近の報告では、新型コロナウイルス感染症(COVID-19)のパンデミック中に、定期小児予防接種の接種率が低下している恐れがあることが示唆されている。パンデミック中に小児予防接種サービスを提供する小児科診療所の対応能力を評価するため、2020年5月12日～20日にVaccines for Children(VFC)プログラムに参加する診療所の調査が実施された。データはサンプリング設計のために加重され、したがって報告されたすべてのパーセンテージが加重された。回答のあった診療所1,933カ所のうち1,727カ所(89.8％)が調査時に診療を行っており、206カ所は休診していた(197カ所は一時的な休診、9カ所は閉院)。診療を行っていた1,727カ所中1,397カ所(81.1％)ではすべての小児患者に、254カ所(14.7％)では一部の小児患者に予防接種サービスを行い、76カ所(4.2％)では行っていなかった。一部の小児患者に予防接種サービスを提供していると回答した診療所では、大半が生後12カ月未満(224カ所、89.2％)および1～2歳(204カ所、81.4％)を対象としており、3～6歳、7～10歳、11～18歳を対象にしているのは半数以下であった。8月までに新規患者の予防接種サービスに対応できる可能性があるかを尋ねたところ、1,933カ所の診療所のうち1,135件(59.1％)が肯定的に回答し、418カ所(21.3％)が可能性は低い/診療所を完全に閉院する/すべての患者を対象にした予防接種は再開しないと回答し、380件(19.6％)が未定であると回答した。都市部の診療所と北東部の診療所では、農村部の診療所や他の3つの地域(中東部、南部、西部)の診療所と比較して、新規患者に対応できる可能性が低かった。これらの結果は、医療提供者が推奨される定期的な小児用ワクチンを提供する能力を持っており、COVID-19に関連した影響の結果として遅延したワクチン接種を補える可能性を示唆している。医療提供者および予防接種プログラムは、両親に予防接種のための受診の必要性を教育し、サービスを提供できない場合は、患者に他の診療を紹介する必要がある。
