Screening for SARS-CoV-2 Infection Within a Psychiatric Hospital and Considerations for Limiting Transmission Within Residential Psychiatric Facilities — Wyoming, 2020

精神病院内でのSARS-CoV-2感染症のスクリーニングと宿泊型精神病施設内での限定的感染に関する考察 ― ワイオミング州、2020年

アメリカでは毎日約18万人が約4,000の精神病院または居住型精神病施設にてメンタルヘルス医療を受けているが、新型コロナウイルス感染症(COVID-19)の原因ウイルスであるSARS-CoV-2はこれら集合住宅環境にて急速に拡大する可能性がある。2020年4月13日、民間の精神病院からワイオミング州立精神病院に患者2例が転院した。この民間の精神病院では入院患者とスタッフにCOVID-19が確診されていた。転院した2例は移送時にはいずれも無症状であり、1例は元の施設でのSARS-CoV-2検査結果が陰性であったが、転入時に隔離と検査が行われた。4月16日に2例とも検査結果でSARS-CoV-2感染が確認された。これを受け、州立病院はCOVID-19の感染防止対策(IPC)を拡大して実施した。新規入院患者のスクリーニング、検査、検査結果が出るまで別室に隔離、陽性の場合は別病棟に隔離し治療、すでに入院している患者に症状を認めた場合も同様の手順を実施することとし、長期療養施設のガイドラインを基にすでに強化されたIPCを実施した。2020年5月1日、州立病院は州保健省およびCDCの支援を得て、患者および医療ケア施設従事者(HCP)におけるSARS-CoV-2感染有病率と新たに実施された患者の入院、隔離の強化、ICP実施の有効性評価のため、点有病率サーベイランスを実施した。サーベイランスには患者76例中46例(61％)、HCP 282名中171名(61％)が参加し、鼻咽頭スワブ検査も実施した。患者は成人精神病科が21/31例(68％)、老人精神病科が16/21例(76％)、刑事司法科が9/24例(38％)、HCPは臨床ケアが137/238名(58％)、室内清掃が14/16名(88％)、輸送/警備が20/27名(74％)であった。患者の入院期間中央値は150日であり、HCPのうち151名(88％)は患者に直接ケアしていた。全例、スワブ検査の結果は陰性であった。これら州立病院での観察と議論、および州の他の施設からの報告に基づき、精神病施設におけるSARS-CoV-2感染の防止と管理に関するさまざまな問題が確認され、解決案が提示されている。また、患者および施設での必要性に合わせ、標準的IPCを適応させることでSARS-CoV-2感染を防止するため、点有病率サーベイランスが有効である可能性が示された。
