Progress Toward Maternal and Neonatal Tetanus Elimination — Worldwide, 2000–2018
妊産婦および新生児における破傷風の根絶に向けた進展 ― 全世界、2000年~2018年
妊産婦および新生児の破傷風(MNT)は依然として公衆衛生上の大きな問題であり、特に予防接種率が低く、清潔な分娩(医療施設または衛生的な場所で医学的訓練を受けた助産師による分娩など)および臍帯処置の利用が限られる地域では、新生児の致死率は80~100%である。1989年に世界保健総会は新生児破傷風(NT)の根絶について是認し、1999年にはイニシアチブが再開され、MNT根絶イニシアチブに改名し、59の優先国を対象とした。根絶戦略には、1)妊婦への定期予防接種と高リスク地域/地区での追加予防接種活動(SIA)を介し、再生産年齢の女性に対し破傷風トキソイド含有ワクチン(TTCV)の2回以上接種率80%以上の達成、2) 分娩の70%以上で熟練した助産師(SBA)によるケアを達成、3)NT症例サーベイランスの強化が含まれた。今回、2000年~2018年におけるMNT根絶達成に向けた進展について要約する。再生産年齢の女性における2回以上のTTCV[破傷風トキソイドの2回接種(TT2+)または破傷風-ジフテリアトキソイドの2回接種(Td2+)]の接種率は、2000年の62%から2018年の72%に増加した。2018年では、59の優先国のうち17カ国(29%)でTT2+/Td2+接種率80%以上が達成された。データが明らかな48の優先国のうち39カ国(81%)で、2000年と比較してTT2+/Td2+接種率が増加した。2018年には、母親が予防接種を受けていた乳幼児の割合は、59カ国の46カ国(78%)において80%以上であった。2018年末までに、59の優先国のうち52カ国(88%)がTTCV SIAを実施し、対象となる再生産年齢女性2億100万人のうち1億5400万人(77%)にTT2+/Td2+を接種した。2000年~2018年の間に、NTの推定死亡数は、170,829例から25,000例に85%減少した。2018年では、新生児の主な死因に占めるNTの割合は1%であり、2000年の7%と比較して大きく減少した。世界中でSBAによる介助を受けた分娩の割合は、2000年~2005年の62%から2013~2018年の81%に増加した。2018年では、データが明らかな51の優先国のうち24カ国(47%)にて70%以上の分娩にSBAが関与した。2018年12月までに、59の優先国のうち45カ国(76%)がMNT根絶を達成していることをWHOにより認証された。残り14カ国での根絶を達成し、達成国での根絶を維持するためには、MNT根絶戦略の実施の維持および強化と、WHOの推奨を受けてTTCVブースター接種を国の予防接種スケジュールに含める必要がある。さらに、妊産婦、新生児、小児に対する保健サービスとワクチン接種サービスの統合、破傷風ワクチン接種場所に行くのが難しい地域を対象とした革新的なアプローチ、サーベイランスを強化するための地域社会の関与も必要である。
