COVID-19 in a Long-Term Care Facility — King County, Washington, February 27–March 9, 2020

長期医療看護施設におけるCOVID-19 ― ワシントン州キング郡、2020年2月27日～3月9日

2020年2月27日、ワシントン州キング郡の高度看護施設(施設A)に入居する73歳の女性が新型コロナウイルス感染症(COVID-19)の症例基準を満たす重度の呼吸器疾患を来し、Public Health-Seattle and King County (PHSKC)に報告された。この女性には冠動脈疾患、インスリン依存状態の2型糖尿病、肥満、慢性腎臓病、高血圧およびうっ血性心不全の既往があり、渡航歴およびCOVID-19感染例との接触はなかった。女性は咳、発熱、息切れなどの症状に対し、施設Aにて5日間の酸素吸入を要し、2月24日、呼吸器症状および低酸素血症が悪化したため、地域の病院へ搬送された。入院時、女性は39.6℃の発熱、頻脈および低酸素血症性呼吸不全を認め、2月25日、挿管および人工呼吸器を要とした。CTスキャンでは両側びまん性肺浸潤を示し、入院4日後に採取した上咽頭、中咽頭および喀痰検体は、いずれも2月28日にSARS-CoV-2陽性が確認され、女性は3月2日死亡した。2月中旬、施設Aでは発熱性呼吸器疾患のクラスターが発生し、数名にインフルエンザ検査が行われていたが、全員陰性であった。PHSKCとCDCはすぐにクラスター調査を開始し、3月9日までにCOVID-19症例を計129例確認した(入居者81例、医療従事者を含む職員34例、訪問者14例)。入居者および職員の発症日は2月16日～3月5日、年齢中央値は入居者が81歳(54～100歳)、職員が42.5歳(22～79歳)、訪問者が62.5歳(52～88歳)、84例(65.1％)が女性であった。入居者の56.8％、職員の5.9％、訪問者の35.7％がCOVID-19により入院し、3月9日時点での死亡率は入居者が27.2％、訪問者が7.1％、職員に死亡例はいなかった。入居者の基礎疾患は高血圧が69.1％、心疾患が56.8％、腎疾患が43.2％、糖尿病が37.0％、肥満が33.3％、肺疾患が32.1％であった。これらを受けて、キング郡にある長期医療看護施設約100件を対象にREDCapを利用した電子メール調査を実施し、3月9日の時点で少なくとも8つの高度看護施設または介護施設にてCOVID-19症例が1例以上確認されている。長期介護施設の入居者ではCOVID-19が急速に蔓延し、重症化および死亡するリスクが高いため、感染の可能性のある職員を特定して施設から退避させ、訪問者を制限し、感染した可能性がある患者を早期に発見し、適切な感染症管理対策を実施するなど、入居者の健康を守り、医療介護の現場を保持するための事前措置を講じるべきである。
