2019新型コロナウイルスの診断を受けた人 ― アメリカ、2020年1月

2019年12月、中国湖北省武漢市にて肺炎の集団症例が発生し、肺炎患者の病因として新型コロナウイルス(2019-nCoV)が特定された。2020年2月4日の時点で、中国では約20,000例の確定診断例を認め、他の23カ国にてさらに159例の確診例が認められている(アメリカの11例を含む)。2020年1月17日、CDCとアメリカ国土安全保障省の税関国境警備局は武漢市から帰国した疾患を有する渡航者を特定するため、空港での健康診断を開始した。また、CDCは1月21日に緊急オペレーションセンターを始動し、2019-nCoV感染が疑われる人に関する照会に対応するプロセスを正式なものとし、1月31日の時点で2019-nCoV感染リスクがあると考えられる約650名の診断を援助するため、公衆衛生局および医療従事者からの臨床的な照会に対応した。発熱と下気道症状(咳、呼吸困難など)の他に加えて、疫学的リスク(武漢市への渡航歴、2019-nCoV感染例または症状のあるPUIとの濃厚接触)を調査対象者(PUI)の判断基準とし、紹介のあった約650名のうち、PUIは210名であった。210名のうち115名(55％)が男性、年齢中央値は29歳であり、医療従事者17名、大学生48名が含まれていた。148名(70％)は旅行関連リスク、42名(20％)は2019-nCoVと確定診断された人またはPUIとの濃厚接触があり、18名(9％)はその両方のリスクがあった。また、6名(3％)が空港での検査、178名(85％)が医療期間、26名(12％)が接触者追跡にて特定された。症状は38℃以上の発熱が143名(68％)、咳/息切れが189名(90％)、上気道症状(咽頭痛、鼻漏、うっ血)が咳/息切れのない9名に認められた。30名はインフルエンザまたはRSウイルスなど他の呼吸器ウイルス病原体検査が陽性であった。入院は42名(20％)、4名(2％)がICUに入院し、1名は通知のあった時に死亡したが、別の死因が確認された(検査結果:陰性)。検査で11名が2019-nCoV感染症と確定診断され、そのうち9名は武漢市に渡航しており、2名はアメリカで感染が確定診断された症例と濃厚接触があった。全例に発熱(自覚的/検温)または咳の症状が出現した。2019-nCoV感染リスクがある人の認識は、症例の特定と、さらなる感染拡大を予防するために重要であり、医療従事者が2019-nCoV感染の可能性がある人を診察するときには、推奨される感染予防法および対策法を怠らずに着実に実施するべきである。
