Update: Characteristics of a Nationwide Outbreak of E-cigarette, or Vaping, Product Use–Associated Lung Injury — United States, August 2019–January 2020

2019年8月以降、CDC、FDA、州および地域の保健部門、公衆衛生および臨床関係者は、電子たばこまたはベイピングなどの製品使用に関連する肺障害(EVALI)の全国的なアウトブレイクについて調査している。今回、National Syndromic Surveillance Programを分析し、州からCDCに報告されたEVALI患者の人口統計学的特性、自己報告による物質使用、入院日、電子たばこまたはベイピング製品に関連した救急外来(ED)受診の分布について最新情報を報告する。2020年1月14日時点で合計2,668例のEVALIによる入院がCDCに報告され、患者の年齢中央値は24歳、66％が男性であった。全体で、EVALI患者の82％がテトラヒドロカンナビノール(THC)を含有する電子たばこまたはベイピング製品(THC含有製品のみを使用した33％を含む)を、57％がニコチンを含む製品(ニコチン含有製品のみを使用した14％を含む)を使用していた。調査では、電子たばこまたはベイピング製品に関連するED受診が、2019年8月に急激に増加し、2019年9月にピークに達した後、減少し続けていることが示されが、臨床医および公衆衛生関係者は、新規のEVALI症例を引き続き警戒すべきである。CDCは、THCを含有する電子たばこまたはベイピング製品について、特に友人、家族、ディーラーなどの非公式な入手先から入手した製品は使用しないことを推奨している。ビタミンE酢酸エステルはEVALIアウトブレイクに強く関連しているため、電子たばこまたはベイピング製品には添加するべきではない。また、一部のEVALI症例の報告では、THC含有または非THC含有製品のどちらでも、他の化学物質が発症に関与している懸念を除外するエビデンスは不十分である。
