Nonadherence to Any Prescribed Medication Due to Costs Among Adults with HIV Infection — United States, 2016–2017
HIV感染症成人における費用を原因とする処方薬のアドヒアランス不良 ― アメリカ、2016年~2017年
アメリカでは処方薬に対する1人当たりの支出が他の高所得国に比べて多く、2017年の患者の自己負担は14%であった。費用の問題による服薬アドヒアランス不良などを含む処方薬のコスト削減法は、疾病率に悪影響を及ぼし、また、HIV患者の場合、感染リスクを高める危険があることから、今回、CDCのMedical Monitoring ProjectがHIV成人患者の行動、治療および臨床転帰について全国的な横断調査を実施した。調査データは2016年6月~2017年5月、対面または電話によるインタビューと診療記録の抽出により収集した。インタビューでは過去12カ月に、1) 医師に低価格の薬について尋ねる、2)他国から処方薬を購入する、3)代替療法を利用する、4)服薬をスキップする、5)服薬量を減らす、6)薬の処方日を遅らせる、などのコスト削減法を行ったかを質問した。その結果、全体でアメリカのHIV成人の約14%は、いずれかのコスト削減対策を行っており、7%がコスト削減のため服薬アドヒアランス不良であった(服薬スキップ:4%、服薬の減量:4%、処方日を遅らせる:6%)。さらに9%が医師に低価格の薬について尋ね、1%が多国から薬を購入し、2%が代替療法を利用した。コスト削減による服薬アドヒアランス不良と年齢、人種/民族、性別、ホームレス、HIV診断からの経過期間との関連性は認めなかったが、世帯収入とは有意に関連していた(貧困レベルより上:8.3%、貧困レベル以下:5.3%)。また、障害がある(障害あり:9.3%、障害なし:5.1%)、また民間医療保険加入者(民間保険加入者:8.3%、民間保険非加入者:5.7%、Medicaid加入者:5.0%)にてコスト削減による服薬アドヒアランス不良が多かった。Ryan WhiteのAIDS Drug Assistance Program(ADAP)を利用していない人では利用者に比較し、有無もコスト削減による服薬アドヒアランス不良が約5倍多かった(利用者:6.7%、必要だが非利用:31.5%、必要なし:5.9%)。また、費用が原因の服薬アドヒアランス不良例では最後のウイルス量検査および過去1年間のすべての検査にてウイルス抑制率が低い傾向にあり(それぞれ64%、55%、服薬アドヒアランス良好例ではそれぞれ76%、68%)、HIV治療をきちんと受ける割合が低く、救急外来受診および入院回数が多かった。これらの調査結果から、今後、ADAPおよびMedicaidの適用を拡大し、民間医療保険費を抑制することでHIV患者の薬剤費コスト削減による服薬アドヒアランス不良を減少し、健康を改善しうることが示唆された。
