Update: Interim Guidance for Health Care Providers for Managing Patients with Suspected E-cigarette, or Vaping, Product Use–Associated Lung Injury — United States, November 2019

CDC、FDA、州および地域の保健部門、公衆衛生および臨床の関係者は、電子たばこまたはベイピング製品使用に関連した肺障害(EVALI)の全国的なアウトブレイクを調査している。CDCは医療従事者に対し、EVALIに関する推奨事項を公表しているが、最近、ユタ州とニューヨーク州の研究者がEVALIの診断および治療アルゴリズムの提案を発表した。現時点では、EVALI診断のための特定の検査やマーカーはなく、EVALIは除外の診断のままで、評価は臨床判断に基づいて行う必要がある。EVALI患者はインフルエンザや他の呼吸器感染症に関連する症状(発熱、咳、頭痛、筋肉痛、疲労など)に似た症状を呈することがあるため、最初の診断でEVALIとインフルエンザまたは市中肺炎を区別することは難しいかもしれず、また、EVALIは呼吸器感染症に併発する可能性がある。今回の報告では、インフルエンザなどの呼吸器感染症がここ数カ月間よりも地域で流行している場合に、EVALIの疑いがある、または既知の患者を治療する医療従事者への推奨事項をまとめた。2019年11月13日現在、49の州、ワシントンDC、および2つのアメリカ領土(プエルトリコおよびアメリカ領バージン諸島)からCDCに2,172例のEVALI症例が報告され、その中に42例(1.9%)のEVALI関連死亡が含まれていた。確立された定義に基づき、EVALI患者の診断には、症状発現の3カ月前以内における電子たばこまたはベイピング製品の使用があり、画像所見が陽性で、感染症によるものは除外する必要がある。最近の試験では、EVALI患者29例の便宜的標本から気管支肺胞洗浄液中にビタミンEアセテートが検出されたが、他の可能性がある化学物質のEVALIへの関与を除外するにはエビデンスは十分ではない。また、大半のEVALI患者は、症状発現前にテトラヒドロカンナビノール(THC)含有製品の使用しているため、CDCは、THCを含む電子たばこまたはベイピング製品を使用しないことを勧めている。CDCの推奨事項は以下のとおりである。1)呼吸器、胃腸、または全身症状がある患者に、電子たばこまたはベイピング製品の使用について質問する。2)臨床的に必要であれば、EVALIが疑われる患者をパルスオキシメトリーや胸部画像診断により評価する。3)一定基準を満たし、臨床的に安定しているEVALI患者には外来治療を考慮する。4)患者に対してインフルエンザ検査を施行し(インフルエンザシーズン中は特に)、抗菌薬や抗ウイルス薬を確立したガイドラインに従い投与する。5)外来患者にコルチコステロイドの処方を検討する際には、外来患者に対する治療法が十分に研究されていないため注意が必要であり、コルチコステロイドにより呼吸器感染症を悪化させる可能性もある。6)患者が電子たばこまたはベイピング製品の使用をやめることを支援するために、行動カウンセリングを含むエビデンスに基づいた治療戦略を推奨する。7)電子たばこまたはベイピング製品を使用する患者を含め、生後6か月以上のすべての人に対して毎年のインフルエンザワクチン接種の重要性を強調する。
