Traumatic Brain Injury–Related Deaths by Race/Ethnicity, Sex, Intent, and Mechanism of Injury — United States, 2000–2017
人種/民族、性別、意図、受傷原因別の外傷性脳損傷に関連した死亡 ― アメリカ、2000年~2017年
外傷性脳損傷(TBI)は、毎年何百万人ものアメリカ人の生活に影響を与えている。さまざまな人種/民族群や性別によるTBIに関連した死亡の傾向を類型化するため、CDCは18年間(2000年~2017年)にわたるNational Vital Statistics Systemの死亡データを分析した。また、外傷を意図的/意図しないによって分類し、さらに受傷原因によって分類した。TBI関連死亡の全体的な割合は2000年~2005年までは一定であり、2005年~2010年にかけて統計的に有意に減少し、その後、2010年~2014年にかけて横ばいとなった。2014年~2017年のTBI関連死亡の全体的な割合は、わずかではあるが統計的に有意に増加した。TBI関連死亡率は、全分析期間を通して全人種の男性で女性よりも有意に高く、年齢で調整した死亡率はアメリカインディアン/アラスカ先住民にて他の人種/民族群よりも有意に高かった。2001年~2006年までは白人と黒人のTBI関連死亡率は同等であったが、2007年以降では白人にて著しく高くなった。2017年では、TBIはアメリカにおいて61,131人の死亡原因となり、この年の約280万人の死亡の2.2%を占めた。2015年~2017年におけるTBI関連死亡の44%が意図的損傷に分類された(殺人、自殺など)。TBI関連死亡で最も多かったカテゴリーは、年代や人種/民族により異なった。たとえば、分析期間内の直近10年間では、自殺がTBI関連死亡の最も大きなカテゴリーとして、不慮の自動車事故を上回った。この変化の一因として非ヒスパニック系白人のTBI関連の自殺による死亡が32%増加したことが影響している。全群におけるTBI関連自殺における受傷原因のほとんどが銃器による受傷であった(97%)。他のTBI関連死亡の主なカテゴリーには、不慮の自動車事故、不慮の転倒、殺人が含まれた。TBI関連死の主要なカテゴリー、特に自殺の大幅な増加を食い止めることを目的としたエビデンスに基づいた予防戦略の広範な実施が必要とされている。医療従事者は、自殺のリスクが高い患者を評価し、適切な介入を提供する上で重要な役割を果たすことができる。
