Global Routine Vaccination Coverage, 2018

2012年の世界保健総会により承認されたGlobal Vaccine Action Plan 2011–2020(GVAP)では、すべての国に対し、2020年までに国の予防接種スケジュールにおける全ワクチンにて90％以上の接種率を達成することを要求している。今回、過去の分析と2018年時点でのWHOおよびUNICEFの世界における推定予防接種率を使用し、全世界、地域別、国別の予防接種率の推定値と傾向を報告する。これらの推算によると、ジフテリア、破傷風トキソイド、百日咳を含有するワクチン(DTP)の初回接種(DTP1)の世界における接種率は、2010年(89％)から2018年(90％)にかけて比較的変化がなかった。2018年ではDTP1の接種率は84％(アフリカ地域)～97％(ヨーロッパ地域)であった。3回目のDTP(DTP3)の世界における接種率は2010年が84％、2018年が86％であり、DTP1と同様に一定していた。2018年、3回のDTPを完遂しなかった小児は世界中で1,940万人いて、そのうち1,350万人(70％)がDTPを1回も接種しておらず、590万人(30％)が開始したがDTPシリーズを完遂しなかった。予防接種のドロップアウト率は、地域、ワクチン、世界銀行の経済分類、GAVIワクチンアライアンスからの支援に該当するかにより異なった。全体として、DTP1からDTP3までのドロップアウト率は2010年の6％から2018年の4％へ低下した。世界における麻疹含有ワクチンの初回接種(MCV1)の接種率は、2010年84％からから2018年86％と維持されていた。2018年のMCV1接種率は74％(アフリカ地域)～95％(西太平洋地域およびヨーロッパ地域)であった。2018年に世界中で118カ国がGVAP 2020の目標であるMCV1国内接種率90％以上を達成し、2017年と同様であった。2回目のMCV(MCV2)を2歳時に接種させている国における接種率は2007年の19％から2018年には54％に増加した。MCV2をより遅い年齢(3～14歳時)に接種させている国でも、接種率は2007年の36％から2018年には69％に増加した。2018年の世界におけるMCV1とMCV2との推定接種率の差は17％であった。ロタウイルスワクチンは、2018年までに101カ国(52％)で導入され、世界におけるロタウイルスワクチンシリーズの接種完遂率は、2010年の8％から2018年には35％と約4倍になった。同期間の世界における肺炎球菌結合型ワクチンシリーズの接種完遂率は11％から47％、風疹ワクチンでは35％から69％、ヘモフィルスインフルエンザ菌b型(Hib)では40％から72％であり、B型肝炎ワクチンの接種率は出生時投与で28％から42％、3回投与シリーズで73％から84％へそれぞれ増加した。小児期、青年期、成人期に推奨されるワクチンの世界的な接種率目標を達成するには、到達が困難および予防接種が困難な集団を対象とするなど、不完全な予防接種に対する決定要因に対処する個々の状況に応じた戦略が必要である。
