Trends in Sexual Risk Behaviors Among Hispanic/Latino Men Who Have Sex with Men — 19 Urban Areas, 2011–2017

ヒスパニック/ラテン系の男性間性的接触者における性的リスク行動の傾向 ― 19の都市圏、2011年～2017年

的確で着実なコンドームの使用とヒト免疫不全ウイルス(HIV)の曝露前予防薬内服(PrEP)は、HIVの性的感染を予防する。アメリカでは、ヒスパニック/ラテン系の男性間性的接触者(MSM)におけるHIV感染症の罹患率は増加しており、HIVリスクは、出身地とアメリカ居住年数によって異なっている。出生地およびアメリカ居住年数によるヒスパニック/ラテン系MSMの性的リスク行動の変化を評価するために、2011年～2017年のCDCのNational HIV Behavioral Surveillanceデータを分析した。今回の分析では、19の都市圏にて3つの横断的なデータ収集サイクル期間に調査した性的に活発なHIV陰性のヒスパニック/ラテン系MSMの計4,731人を対象とした(2011年：1,581人、2014年：1,479人、2017年：1,671人)。過去1年間における全体でのコンドームなしの肛門性交経験率は2011年の63％から2017年には74％へ増加し、PrEP使用率は2014年の3％から2017年には24％へと増加した。2017年では、過去1年間のPrEPの使用は、アメリカ生まれのヒスパニック/ラテン系MSMでは1024人中283人(28％)で、アメリカ以外で出生したMSMではアメリカ居住期間が6年以上で457人中87人(19％)、5年以下で188人中30人(16％）であった。2017年におけるコンドームなしの肛門性交経験率は、出生地および居住年数別のすべてのグループで同様であった(73.8％～74.6％）。また、2017年におけるコンドームもPrEPも使用していない過去1年間の肛門性交経験率は、アメリカ生まれのヒスパニック/ラテン系MSM(49％)がアメリカ以外で出生したMSMよりも少なかった(居住年数6年以上：58％、5年以下：59％)。以上、ヒスパニック/ラテン系MSM、特にコンドームなしの肛門性交を行うMSMのHIV感染を減らすためには、PrEPが重要なツールになり得ることが示された。PrEP服用などのHIV感染予防のための介入には、ヒスパニック/ラテン系MSMの文化的および言語的ニーズに対応する必要がある。
