ポリオウイルス封じ込めの実施に向けた進展 ― 全世界、2018年～2019年

3種類の野生株ポリオウイルス(WPV)のうち、2型WPVは世界中で根絶したと2015年にGlobal Commission for the Certification of Poliomyelitis Eradication(GCC)によって宣言された。その後、2016年には、Sabin 2株の経口ポリオワクチン(OPV)の定期接種は世界中で中止され、同時に3価OPV(ワクチンウイルス1型、2型、3型を含有)から2価型OPV(1型と3型を含有)への切り換えが実施された。WPV3型(WPV3)は2012年を最後に検出されておらず、2019年後半に根絶が宣言される可能性がある。いったん世界的なポリオ根絶が達成され集団ワクチン接種キャンペーンが実施されなくなれば、ポリオウイルスに対する集団の免疫力は低下していくため、施設での封じ込めが侵害され地域社会へポリオウイルスが漏出した場合は深刻な結果をもたらす。このリスクを軽減するため、2015年の第68回世界保健総会でWHO加盟194カ国により、全種のポリオウイルスが特別に承認されたポリオウイルス封じ込め施設のみで保管されることが決議された。型別でのWPV根絶と経口ポリオワクチン使用の順次停止後におけるポリオウイルス保管施設関連でのリスクを最小限に抑えるための2015年に発表されたWHO Global Action Plan改訂版(GAPIII）では、研究所、ワクチン製造所、その他のポリオウイルス保管施設に対するバイオリスク管理のための概要を述べている。ポリオウイルス2型(PV2)物質から始まるポリオウイルス封じ込めへの段階的アプローチで、フェーズⅠでは、これらの物質を保管している施設を特定し施設数を減少させるために、国内の施設調査に焦点を当て、フェーズⅡでは、PV2物質を保管しているPEF(ポリオウイルスを管理する最小限の施設)すべての認証に関連する活動の概要の説明、フェーズⅢでは、すべてのタイプのWPV、ワクチン由来ポリオウイルス(VDPV)、およびOPV/Sabinポリオウイルス株の最終的な封じ込めが含まれる。2019年8月1日現在、26カ国にある74のPEFにてPV2物質が保管されている。これらの国のうち、25カ国がポリオウイルス封じ込めに関する国家封じ込め認証機関(NAC)を設立し、封じ込めの認証の責任を負う保健省または同等の機関に指定している。指定されたすべてのPEFは、2019年12月31日までに認証プロセスへの登録を要求されている。GCCがWPV3根絶を認定する場合、WPV3およびVDPV3型物質も含める必要があり、指定されたPEF数が一時的に増加することになる。封じ込め条件を必要としない野生株およびOPV/Sabin株に対する安全な代替品が診断および血清学的検査に利用できる場合、PEF数は減少する可能性がある。世界的な根絶後もポリオウイルスを保管する施設は、効果的なバイオリスク管理手法を採用することにより、コミュニティへの再導入のリスクを最小限に抑える必要がある。
