Racial/Ethnic Disparities in HIV Preexposure Prophylaxis Among Men Who Have Sex with Men — 23 Urban Areas, 2017
男性間性交渉者のHIV曝露前予防における人種/民族による格差 ― 23都市部、2017年
2017年の予備データでは、ヒト免疫不全ウイルス(HIV)感染症の新規診断例の67%はゲイ、バイセクシュアル、男性間性交渉者(MSM)が占めており、静注ドラックを使用するMSMが3%を占め、アフリカンアメリカン/黒人(黒人)およびヒスパニック/ラテン系(ヒスパニック系)MSMにて不均衡に発症している。2010年から2015年にかけてMSMにおけるHIV発症率の人種/民族的格差は拡大し、2015年では白人MSMの発症率に比べて黒人は10.5倍、ヒスパニック系は4.9倍となっている(2010年はそれぞれ9.2、3.8倍)。処方通り服用することで性交渉によるHIV感染のリスクを約99%抑制する曝露前予防内服(PrEP)の利用が増加すれば、これらの格差が縮小し、「Ending the HIV Epidemic:A Plan for America initiative」を後押しすると思われる。今回、PrEPについての認知度、医療関係者と話したことがあるか、利用における人種/民族的格差を把握するため、CDCは2017年のNational HIV Behavioral Surveillanceデータから、アメリカ23都市部のMSMについて分析した。2017年に23都市部で面談した性的に活発なMSM 計10,104名の中から、HIV検査陰性であり、PrEPが臨床適応となりそうなMSM4,056名(白人:1,843名、ヒスパニック系:1,251名、黒人:962名)を今回の分析対象とした。その結果、PrEPを認知していたのは白人1,742名(95%)、ヒスパニック系1,088名(87%)、黒人825名(86%)であり、白人と比較したヒスパニック系の推定調整有病率(aPR)は1.1、白人と比較した黒人のaPRも1.1であった。過去1年以内に医療関係者とPrEPについて話をしたことがあるのは白人1,063名(58%)、ヒスパニック系546名(44%)、黒人412名(43%)のみであり(aPRはともに1.2)、さらに、過去1年以内にPrEPをした人は白人765名(42%)、ヒスパニック系373名(30%)、黒人248名(26%)のみであり(aPR:ヒスパニック系1.2、黒人1.4)、白人MSMはヒスパニック系、黒人に比べPrEPを認知しており、医療関係者と話したことがあり、服用した割合が有意に多かった。医療関係者とPrEPについて話したことがあるMSMのうち、実際に服用した人の割合は白人68%(724/1,063名)、ヒスパニック系62%(338/546名)、黒人55%(225/412名)であった。また、白人と黒人の格差は医療保険加入者および日常的に医療ケアを使用している人にもあり(aPRはともに1.2)、アメリカ国勢調査地域別では、白人と黒人との格差は南部(アトランタ、ボルチモア、ダラス、ヒューストン、マイアミ、ニューオーリンズ、バージニアビーチ、ワシントンDC)および西部(デンバー、ロサンゼルス、ポートランド、サンディエゴ、サンフランシスコ、シアトル)にて認められ(aPR:南部1.2、西部1.3)、白人とヒスパニック系との格差は南部のみに認められた(aPR:1.2)。以上、HIV予防にはすべてのMSMに対しPrEPをするように促進し、人種/民族による格差なくすことを目標とする必要性が示された。
