Impact of Public Health Interventions on Drinking Water–Associated Outbreaks of Hepatitis A — United States, 1971–2017
飲料水に関連するA型肝炎のアウトブレイクに対する公衆衛生的介入の影響 ― アメリカ、1971年~2017年
A型肝炎ウイルス(HAV)は主に糞口経路にて感染し、ごくまれに感染者に肝不全や死亡を引き起こす。アメリカでは飲料水に関連するA型肝炎アウトブレイクの報告は稀であり、1971年~2008年ではほとんどが未処理地下水によるアウトブレイクであった。今回、1971年~2017年にWaterborne Disease and Outbreak Surveillance Systemに報告された飲料水に関連するA型肝炎アウトブレイクについて分析した。この期間のアウトブレイクは18州より32件(計857例)報告され、最後が2009年であり、死亡例は認めていない。給水系別では個人(個人所有の井戸、泉など)13件(41%) /257例(30.0%)、地域共同体10件(31%)/241例(28.1%)、地域共同体以外9件(28%) /359例(41.9%)であり、水源は地下水30件(94%)/804例(93.8%)、未処理水23件(72%)/585例(68.3%)であった。飲料水によるA型肝炎アウトブレイクは、1989年のU.S. Environmental Protection AgencyによるTotal Coliform and Surface Water Treatment Rules、および1996年のAdvisory Committee on Immunization PracticesによるHAVワクチン接種推奨により有意に減少し、前者では1971年~1989年の24件から1990年~2017年には8件(77%減少)、後者では1971年~1996年の29件から1997年~2017年には3件(87%減少)となった。1995年~2009年に発生した飲料水によるA型肝炎アウトブレイク4件(35例)はすべて未処理地下水を水源とした個人の給水系が原因であり、2009年7月以降、水に関連するA型肝炎アウトブレイクは報告されていない。公衆衛生当局は、個人の井戸を使用する場合の未処理地下水によるリスクを認識し、水質検査および処理によりHAV感染を防止することが重要である。
