Outbreak of Salmonella Newport Infections with Decreased Susceptibility to Azithromycin Linked to Beef Obtained in the United States and Soft Cheese Obtained in Mexico — United States, 2018–2019
アメリカで入手した牛肉とメキシコで入手したソフトチーズに関連し、アジスロマイシンに対する感受性が低下したSalmonella Newport感染症のアウトブレイク ― アメリカ、2018年~2019年
2018年9月にCDCは、経口治療薬であるアジスロマイシンへの感受性が低下して多剤耐性のサルモネラ菌血清型Newport感染症を確認、2018年6月~2019年3月に国内32州で255例を確認した。入院情報が判明しているうち、29%(60例/209例)の患者が入院、6%(4例/70例)がICUに入院し、4%(10例/255例)がサルモネラ菌血症を来し、2例が死亡した。患者の年齢中央値は36歳(1歳未満~90歳)、58%(145例/250例)が女性、65%(143例/221例)がヒスパニック系であった。全体で43%(89例/206例)が発症前7日間の間にメキシコへ旅行しており、そのうち食品の摂取状況について情報が得られた中の87%(41例/47例)が牛肉、63%(29例/46例)がソフトチーズを食していた。チーズはケソフラスコ(牛や山羊の低温殺菌していない乳から作る生チーズ)がもっとも多かった。メキシコへ旅行していない患者では29%(20例/70例)がメキシカンスタイルのソフトチーズ、93%(68例/73例)が牛肉を食していた。2018年9月、アウトブレイク株はNational Antimicrobial Resistance Monitoring Systemサーベイランスの一環としてテキサス州の屠殺加工工場で採取された去勢仔牛の盲腸検体から検出され、2018年10月にはメキシコのティフアナにあるマーケットで購入されたケソフラスコとオアハカチーズのミックスチーズから検出された。このチーズは発症した患者がアメリカに持ち込んだものであり、患者の菌株はチーズから分離した株と区別できなかった(コアゲノム多座塩基配列タイピングの差異0アレル)。アウトブレイク株は2018年11月および2019年3月にテキサス州の屠殺加工施設で採集された牛肉の試料から検出され、メキシコのチーズ、去勢仔牛の盲腸および牛肉から分離された株は0~2アレルの差異であり、患者から分離された株と0~1アレルの差異であった。全ゲノムシークエンスでは、耐性情報のある252分離株のうち、226株(90%)はトリメトプリム/スルファメトキサゾール、テトラサイクリン、クロラムフェニコールに耐性、アジスロマイシンへの感受性の低下が予測された。また、143株(57%)ではさらにアンピシリンおよびストレプトマイシン耐性、シプロフロキサシン非感受性が予測された(最小発育阻止濃度0.12μg/mL以下)。患者の治療情報では65例/87例(75%)に抗生物質治療が行われ、28/86例(33%)が投与された抗生物質はアウトブレイク株に対し耐性または感受性低下を示すものであった。消費者は低温殺菌処理されていない牛乳から作られたソフトチーズを食べることは避け、牛肉を調理する際には適切な温度に達したことを確認するため料理用温度計を使用し、抗生物質の投与が必要な場合は可能な限り感受性検査に基づいて薬剤を選択するのがよいと思われる。
