Update: Influenza Activity in the United States During the 2018–19 Season and Composition of the 2019–20 Influenza Vaccine

2018-19年シーズン(2018年9月30日~2019年5月18日)のアメリカにおけるインフルエンザ活動性は、中程度の重症度であった。全国的に、インフルエンザ様疾患の活動は11月に増加し始め、2月中旬にピークに達し、4月中旬にベースラインを下回った。シーズンは21週間続き、10年間で最も長いシーズンとなった。公衆衛生研究所は2018年9月30日から2019年5月18日の間に80,993検体を検査し、42,303検体(52.2%)がインフルエンザウイルス陽性であり、そのうち40,624検体(96.0%)がインフルエンザA、1,679検体(4.0%)がインフルエンザBに対して陽性であった。38,995件の季節性インフルエンザAウイルスのサブタイプは、インフルエンザA(H1N1)pdm09が22,084件(56.6%)、インフルエンザA(H3N2)が16,991件(43.6%)であった。インフルエンザB系統の情報は1,105件(65.8%)が入手可能で、そのうち406件(36.7%)がB/Yamagata系統、699件(63.3%)がB/Victoria系統であった。インフルエンザA(H1N1)pdm09ウイルスは、2018年10月から2019年2月中旬にかけて全国的に流行しているウイルスの大部分を占めたが、インフルエンザA(H3N2)ウイルスは、全国的に2月下旬から2019年3月末までのアメリカのHealth and Human Services(HHS)の 10地域すべてで、A(H1N1)pdm09ウイルスよりも頻繁に検出された。シーズン全体では、インフルエンザA(H3N2)ウイルスがHHS地域4、6、7で優勢であり、残りの7地域では、インフルエンザA(H1N1)pdm09ウイルスが優勢であった。合計2,699件のインフルエンザウイルス検体をオセルタミビル、ザナミビル、およびペラミビルに対する耐性について検査した結果、5件(0.3%)のインフルエンザA(H1N1)pdm09ウイルスではアミノ酸置換(H275Y)を有し、オセルタミビルおよびペラミビルによる阻害が非常に低下していた。さらに、4件(0.3%)のインフルエンザA(H1N1)pdm09ウイルスでは、オセルタミビルによる阻害が多少減少し、また、異なる系統からの2つのインフルエンザBウイルス(0.4%)では、アミノ酸置換(H273Y)を有し、ペラミビルによる阻害が大きく減少した。検討した2,673件のインフルエンザウイルス検体の中では、これまでにバロキサビルに対する感受性の低下と関連していたPAタンパク質のアミノ酸置換は含まれていなかった。WHOとFDAのワクチンおよび関連生物製剤諮問委員会は、北半球の2019-20年インフルエンザワクチン組成成分として、インフルエンザ三価ワクチンではA/Brisbane/02/2018 A(H1N1)pdm09様ウイルス、A/Kansas/14/2017 A(H3N2)様ウイルス、およびB/Colorado/06/2017様(B/Victoria系統)ウイルスを含有することを推奨した。また、四価ワクチンでは、三価ワクチンウイルスとB/Phuket/3073/2013様(B/Yamagata系統)ウイルスを含有することを推奨した。2017-18年のインフルエンザシーズンと比較した今シーズンの入院率は、成人では低かったが、小児では同程度であった。インフルエンザ活動性は現在サーベイランスの基準値を下回っているが、季節性インフルエンザウイルスの検査と新型インフルエンザA型ウイルス感染の監視は年間を通して継続するべきである。季節性インフルエンザワクチンを毎年接種することは、季節性インフルエンザとその重症な転帰の可能性を防ぐために最善の方法である。
