麻疹根絶に向けた進展 ― パキスタン、2000年~2018年
1997年、WHOの東地中海地域(EMR)の21カ国が、麻疹を根絶するために東地中海地域委員会の第41回会合中に決議を可決し、2015年、この目標はEastern Mediterranean Vaccine Action Plan 2016-2020の優先事項として盛り込まれ、第62回地域委員会で承認された。麻疹の根絶を達成するために、WHO東地中海地域事務所は次の4本の柱からなる戦略、すなわち1)定期予防接種サービスを介し、各国全地区の小児に対する麻疹含有ワクチン(MCV)の初回接種率95%以上の達成、2)定期的な2回目ワクチン接種スケジュールの実施または追加予防接種活動(SIA)のいずれかを通じて、各国全地区でMCV2回目の接種率95%以上の達成、3)すべての国で質の高い、症例ベースの麻疹サーベイランスの実施、4)ビタミンAのサプリメントを含む、最適な臨床麻疹症例に対する管理の提供、を策定した。人口が約2億人のパキスタンは、EMR全人口の3分の1近くを占める。今回、2000年~2018年のパキスタンでの麻疹根絶に向けての進展と課題について述べる。調査期間中、初回MCV投与(MCV1)の推定接種率は2000年の57%から2017年には76%に増加した。2回目のMCV投与(MCV2)は2009年に全国で導入され、MCV2接種率は2009年の30%から2017年には45%に増加した。2000年~2018年の間に、約2億3250万人の小児がSIA中にMCVの接種を受けた。報告された麻疹罹患率は、2000年~2009年の100万人あたり平均24.6例から2010年~2018年に平均80.4例に増加し、2013年(230.3例)と2018年(153.6例)にピークがあった。2017年と2018年に麻疹疑い例で調査後に麻疹ではないとして除外された症例の割合は、症例の過少報告を反映して、人口10万人あたり各年2.1例および1.5例であった。2007年~2018年に麻疹症例と確定診断された201例の遺伝子型(D4:50例、B3:150例、H1:1例)が得られ、D4型は2007年~2013年に認められており、B3型は2010年~2018年の世界的な蔓延とともに2011年~2018年に優勢であった。麻疹の根絶を達成するために、MCV1およびMCV2接種率の増加、予防接種サービスを利用していないコミュニティを特定し、利用できるようにするための戦略の策定、すべての地区で症例ベース麻疹サーベイランスの感度向上がさらに必要である。
