麻疹撲滅への進歩 ― ヨーロッパ地域、2009年～2018年

2010年、WHOヨーロッパ地域(EUR)全53カ国は麻疹、風疹および先天性風疹症候群を撲滅する方針の再確認を行った。2002年以降、EUR 53カ国は小児の定期的ワクチン接種スケジュールに麻疹含有ワクチン(MCV)の2回接種を導入しており、2009年～2017年における地域別の初回MCV(MCV1)および2回目MCV(MCV2)の推定接種率はそれぞれ93％から95％、73％から90％へ増加した。2017年、MCV1接種率≧95％を達成した国は30カ国(57％)、MCV2≧95％は15カ国(28％)であり、この期間、1,600万人以上が13カ国、21回の追加接種活動(SIA)を通じて接種を受けた。麻疹サーベランスデータはEUR諸国からWHOへ直接またはEuropean Centre for Disease Prevention and Controlを介して報告される。2018年の時点で47カ国(89％)が症例ベースのサーベランスデータ、6カ国(11％)が集計データを報告している。2009年～2018年、年間地域麻疹発症率は2009年の100万人あたり8.8(7,884例)～2010～2015年の平均30.1(平均28,021例)と差があり、2016年には低下し[5.8(5,273例)]、2018年には約14倍[89.5(82,596例)]増加した。この82,596例はEURの47カ国(89％)から報告され、そのうち73,295例(89％)はウクライナ(53,218例、64％)、セルビア(5,076例、6％)、フランス(2,913例、4％)、イスラエル(2,919例、4％)、ジョージア(2,203例、3％)、ギリシャ(2,193例、3％)、イタリア(2,517例、3％)、ロシア(2,256例、3％)の8カ国から報告された。もっとも高い発症率は2018年、ウクライナにて100万人あたり1,209.2、セルビアにて579.3であった。2018年に報告された麻疹症例のうち、20歳以上の成人は30,561例(37％)を占め、イタリア(68％)、セルビア(67％)、ロシア(42％)にて成人症例の割合が大きかった。2009年～2018年に179例の死亡が報告され、うち114例(64％)は2017年～2018年に生じ、ルーマニア(46例)、ウクライナ(20例)、セルビア(15例)、イタリア(12例)にて多かった。また、ウイルス配列分析は17,587検体にて行われ、検出された主な遺伝子型はD4(全体の21％、2009年～2012年では66％)、D8(全体の45％、2013年～2016年では76％)、B3(全体の33％、2017年～2018年では58％)であった。2017年の年末時点で43カ国(91％)が麻疹ウイルスの伝播が12カ月以上認められておらず、うち37カ国(70％)は36カ月以上認めていない。
