2018年10月、Advisory Committee on Immunization Practicesが発表した19歳以上の成人の2019年の予防接種スケジュールについて、2018年からの変更点がまとめられた。インフルエンザワクチンは2018年6月、2016-17年期および2017-18年期後に弱毒化生インフルエンザワクチン(LAIV)の使用を推奨している。2018-19年期は患者の年齢と健康状態により推奨されるすべての認可ワクチンの接種が可能である。LAIVは49歳以下(HIV感染症を含む免疫不全状態、解剖学的または機能的無脾症、妊婦、保護環境下での重度免疫不全症患者の接触者または介護者、48時間以内にインフルエンザ抗ウイルス剤の投与を受けた人、脳脊髄液漏れまたは人工内耳装着者を除く)に接種され、前回の接種から6週間以内にギランバレー症候群の既往がある場合は接種すべきではない。B型肝炎ワクチン(HepB)に関しては、2018年2月、ACIPはシトシン-リン酸-グアニン1018オリゴデオキシヌクレオチドアジュバントを用いた新規の単抗原リコンビナントB型肝炎ワクチン(Heplisav-B、2017年11月にFDA承認)の使用を推奨した。Heplisav-Bは4週間以上の間隔を空け、2回接種される。妊娠中の安全性に関するデータがないため、妊婦への接種は避けるべきである。A型肝炎ワクチン(HepA)は2018年10月、単抗原ワクチン(Harvix)の2回接種またはA型B型肝炎混合ワクチン(Twinrix)の3回接種の適応対象にホームレスが追加された。他に感染リスクの高い集団として、1)慢性肝疾患または凝固因子障害のある人、2)A型肝炎の流行が高度または中等度の国々へ旅行する人、3)A型肝炎の流行国からの国際養子縁組者に入国60日以内に接触する人、4)男性同性愛者、5)静注または非静注ドラッグを使用する人、6)研究室でA型肝炎ウイルスを扱う人またはウイルスに感染した霊長類を扱う人への接種が推奨され、さらに感染のリスクはないが感染予防を希望する人にも接種される。2018年からの変更点については、表紙(成人に推奨される接種スケジュール)では、CDC Vaccine Schedules Appのダウンロードおよび症例固定やアウトブレイクへの対応などを含むサーベイランスに関する参考資料へのアクセスリンクが表示され、また、ワクチンにて予防可能な疾患が疑われる症例や接種後の有害事象に関する報告の指示が簡素化された。表1(年齢別に推奨される接種スケジュール)では49歳以下の成人に対するLAIVのリストが不活化インフルエンザワクチン(IIV)とリコンビナントインフルエンザワクチン(RIV)と別々に表示された。表2(医学的状態により推奨される接種スケジュールおよび他の適応症)では、LAIVがIIVおよびRIVと別々に示され、一部のワクチンはオレンジ色(慎重投与:有害反応のリスクを予防による利益を上回る場合に接種される)とピンク色(ワクチンが必要な場合は妊娠後まで延期する)にて表示された。妊婦またはHIV感染を含む免疫不全状態の成人に対するLAIVの使用は禁忌として赤色で表示し、解剖的または機能的無脾症または補体不全症に対するLAIV接種による有害事象リスクは不明であるが、禁忌に分類され、末期の腎臓、心臓および肺疾患、慢性肝疾患、糖尿病患者に対するLAIVの接種は慎重投与に指定されている(オレンジ色)。妊婦に対する髄膜炎菌Bワクチン(MenB)接種は慎重投与(オレンジ色)とされ、髄膜炎菌A、C、W-135およびY結合型ワクチン(MenACWY)は追加のリスク因子または他の適応のある成人に対し推奨されるワクチン(紫色)に指定されている。さらにヒトパピローマウイルスワクチン(HPV)、リコンビナント帯状疱疹ワクチン(RZV)は妊娠後まで遅らせる(ピンク色)とされ、弱毒化生帯状疱疹ワクチン(Zostavax)の接種は妊娠中には禁忌である。
