人種と民族性によるHIVの曝露前予防 ― アメリカ、2014年～2016年

抗レトロウイルス薬を含有する経口薬の毎日の服用による曝露前予防(PrEP)は、ヒト免疫不全ウイルス(HIV)感染を予防する上で非常に有効である。フマル酸テノホビルジソプロキシル(TDF)とエムトリシタビン(FTC)の併用は、PrEPとしてFDAに承認された唯一の薬物治療である。PrEPは、HIV感染のリスクを増大させる性行動または注射薬の使用行動を持つ男性および女性に適応される。CDCは、IQVIA社のデータベースであるリアルワールドデータの Longitudinal Prescriptionsから2014年～2016年のデータを分析し、米国におけるPrEPを処方した人数(使用者)を推定し、性別および人種/民族性を含む人口統計的特徴をまとめた。16歳以上のPrEPの年間使用者数は、2014年の13,748人から2016年の78,360人に470％増加した。2016年では、PrEP使用者の65.0％が25～44歳で、0.1％が16～17歳で、男性は全PrEP使用者の95.3％を占めていた。PrEP使用者の割合は、国勢調査地域の西部(29.7％)、南部(27.2％)、北東部(26.7％)で高く、中西部(16.3％)で最も低かった。 PrEP使用者の医薬品の支払者は、81.0％が商業健康保険、12.2％がMedicaidであった。医薬品支援プログラムの恩恵を製造会社から受け取ったPrEP使用者数は、2014年の435人から2016年の5,437人に大幅に増加した。PrEPのためのTDF/FTC処方薬の使用期間が30日超ではなく28日と定義された場合、2016年のPrEP使用者の総数は78,360人から98,599人に26％増加した。両方のアルゴリズムを使用した人口統計および支払者タイプの分布は、同様であった。2016年に確認された78,360人のPrEP使用者のうち、人種/民族の情報が利用可能であったのは32,853人(41.9％)で、白人は22,574人(68.7％)、黒人は3,687人(11.2％)、ヒスパニック系は4,317人(13.1％)、アジア人は1,486人(4.5％)であった。性別によって層別化した場合、人種/民族データのある1,146人の女性PrEP使用者のうち、白人は554例(48.3％)、黒人は297例(25.9％)、ヒスパニック系は201例(17.5％)であった。黒人男性と女性はPrEP適応の人の約40％を占めていたが、この調査では、PrEPを処方されていた白人男性と女性は黒人男性と女性の約6倍であった。特に、女性、黒人、ヒスパニック系の人では、PrEP適応のある人の人数とPrEPを処方された人の人数の差が大きく、アメリカにおけるPrEPの影響を高めるためには、集中的な取り組みが必要である。
