ポリオウイルス抑制の実現に向けた進捗 ― 全世界、2017年～2018年

1988年に世界保健総会(WHA)がポリオ根絶を決議した後、3種類の野生型ポリオウイルス(WPV)のうち、2型(WPV2)は2015年に根絶が宣言され、3型(WPV3)は2012年以降報告されておらず、1型(WPV1)は2017年、2018年、アフガニスタンとパキスタンのみにて報告されている。世界的なポリオ根絶後、ウイルスを保有する施設は地域へのウイルスの再導入および伝播の再確立リスクを最小限に抑える必要があり、ポリオの封じ込みには実験室、ワクチン製造施設、ウイルス保有施設などのバイオリスク管理が重要となる。ここでは前回(2017年)報告されたポリオ抑制活動の進捗状況について報告する。2017年のポリオ症例数はアフガニスタンとパキスタンから報告されたWPV1による22例であり、ナイジェリアからは報告されていない。2015年のWPV2根絶後、2016年2月、ワクチンが3価から2価に変更となり、多くの国々で大きな問題なく移行したが、ワクチン由来2型ポリオウイルスが出現、アウトブレイクコントロールのため11カ国(カメルーン、チャド、コンゴ民主共和国、エチオピア、ケニア、モザンビーク、ニジェール、ナイジェリア、パキスタン、ソマリア、シリア)にて1価2型ワクチン1,260万回分が提供され、2018年もコンゴ民主共和国、アフリカの角(エチオピア、ケニア、ソマリア)およびナイジェリア北部にてアウトブレイクが発生し、備蓄ワクチンの使用が要求されている。WHOのGlobal Action Plan (GAPIII)は、WPV根絶および経口ワクチンの使用中止後、ポリオウイルスを扱う施設に関連するリスクを最小限にするためのバイオリスク管理基準であり、WHO加盟国は2019年4月までに指示された項目を実施し、PV2試料に関する報告を完了することが求められている。2018年5月の第71回WHAにてWHO加盟国はGAPIIIが要求するこれらの事項に迅速に対応することを採択し、2018年8月の時点で29ヵ国にて政府がPV2試料を保管する81のpoliovirus essential facilitiesを計画し、うち22カ所はそれぞれのnational authorities containment(NAC)を確立している。NACは施設の監査や抑制の結果を認証する権限を有するものであり、WHOはNACの監査能力を強化し、国際GAPIII審査員を確保するための訓練を実施している。
