Assessment of State, Local, and Territorial Zika Planning and Preparedness Activities — United States, June 2016–July 2017
州、地方および領土におけるジカに対する計画と準備活動の評価 ― アメリカ、2016年6月~2017年7月
ジカウイルス疾患に対する緊急対応には国、州、地方および領土における公衆衛生管轄区域間の協力が必要であり、CDCは2016年1月21日、緊急対策センターを始動させ、7つの特別部会により国の対策を支援した。また、2016年7月、53の管轄区域[41州、8領土(アメリカ領サモア、ミクロネシア連邦、グアム、マーシャル諸島、北マリアナ諸島、パラオ、プエルトリコ、アメリカ領ヴァージン諸島)、4大都市圏(シカゴ、ロスアンゼルス郡、ニューヨーク市、ワシントンDC)]に対し、ジカウイルスに対する準備と対応のため補助金2,500万ドルを支給し、さらに12月、リスクの高い21区域に2,500万ドルを追加支給した。資金供給を受けた管轄区域はCDCガイドラインに記載された項目に対処し、四半期ごと[2016年6月(baseline)、10月、2017年1月、4月、7月]にチェックリストに回答することが求められた。チェックリストは対応を3ステージ(1:発現前、2:発現が疑われる/確認、3:発現/対応)、4段階(Phase 0:準備段階、Phase 1:蚊のシーズン、Phase 2:局所的な伝播確定、Phase 3:局所的な複数例での伝播確定)に分類し、ガイダンス要素は7項目(1:作戦と計画、2:情報の伝達および教育、3:ベクターコントロール、4:サーベイランス、5:研究室検査、6:妊婦へのアウトリーチ、7:血液の安全性)を評価した。2016年6月および7月において、局所的な蚊媒介感染を複数例確認した区域はそれぞれ3、7区域であり、10月までにすべての区域にて症例が報告された。Phase 0~1にて85%以上のガイダンス要素を実行している区域は53区域中77%(項目1)~98%(項目2および6)であり、Phase 2~3では10区域中71%(項目3および6)~100%(項目1、4、5および7)であった。ジカウイルスに対する準備、対応および復旧計画が確立している区域は2016年6月:14(26%)、2017年7月:34(64%)であり、項目6は24(45%)から46(87%)、項目2は31(58%)から51(96%)、項目3は17(32%)から30(57%)に増加していた。2017年7月に局所的な伝播を認める7区域(アメリカ領サモア、フロリダ州、ミクロネシア連邦、プエルトリコ、マーシャル諸島、テキサス州およびアメリカ領ヴァージン諸島)では、各項目に対応するとともにアウトリーチ計画を作成している。
