Occupational Patterns in Unintentional and Undetermined Drug-Involved and Opioid-Involved Overdose Deaths — United States, 2007–2012
非意図的な、および原因不明な薬物またはオピオイドが関与する過剰摂取による死亡の職業的パターン ― アメリカ、2007年~2012年
オピオイドの蔓延は米国人口の複数の階層に影響を及ぼしており、職業パターンはオピオイド蔓延を理解するために重要である可能性がある。 オピオイドは、職業関連の特定の外傷タイプに対して処方されることがあるが、職業は多岐にわたる。 CDCは26の職業グループ内において非意図的または原因不明の薬物過剰摂取による死亡率を調べるために、National Occupational Mortality Surveillanceシステムの死亡データを使用した。 この調査には、2007年~2012年のアメリカ21州のデータが含まれた。 薬物過剰摂取による死亡率を年齢、性別、人種、年齢、および州について間接的に標準化された特定死因死亡比(PMR)を使用して総死亡率と比較し、オピオイド関連の薬物過剰摂取による死亡に特有の死亡パターンも調査した。今回の分析では、試験集団内に57,810例の薬物過剰摂取による死亡が確認された(死亡4,024,086名のうちの1.4%)。 薬物過剰摂取による死亡率は、男性(61.8%)、白人(89.8%)、45~54歳(30.1%)または35~44歳(24.1%)の集団で高かった。 薬物過剰摂取のPMRは、以下の6つの職業グループ:1)建築業(1.25)、 2)採掘業(1.16)、3)食品調理および給仕(1.11)、4)医療従事者および技術者(1.16)、5)医療補助(1.18)、6)介護とサービス(1.10)で、有意に1.00を上回った。また薬物過剰摂取のPMRは、通常の職業が給料未払/失業(1.10)または不明(1.31)の死亡者でも有意に高かった。建設業は、薬剤過剰摂取による死亡とヘロイン関連および処方されたオピオイド関連の両方の死亡でPMRが最高値を示した。メタドン、天然および半合成オピオイド、メタドン以外の合成オピオイドのPMRがもっと高い職業グループは、建設業、採掘業、医療従事者であった。建設業グループ内の18歳以上の薬物過剰摂取による死亡(7,402例)を分析した結果、 死亡者の大多数は男性(96.7%)、白人(92.6%)、45~54歳(30.4%)、35~44歳(26.9%)であった。 以上、職場は大多数のアメリカ成人にとって生活に不可欠な要素であることから、職場の研究および介入を取り入れることは、オピオイド蔓延の対応に有益と考えられる。
