National, Regional, State, and Selected Local Area Vaccination Coverage Among Adolescents Aged 13–17 Years — United States, 2017

国家、地区、州、選択した地域別の13～17歳の若者におけるワクチン接種率 ― アメリカ、2017年

Advisory Committee on Immunization Practices(ACIP)では、11～12歳を対象としたヒト・パピローマウイルス(HPV)ワクチン、4価髄膜炎菌結合型ワクチン(MenACWY)、破傷風・弱毒化ジフテリア・無菌性百日咳ワクチン(Tdap)の定期予防接種、16歳でのMenACWYの追加投与、小児期に予防接種済み(UTD)ではない若者に対するB型肝炎ワクチン、麻疹、流行性耳下腺炎、風疹ワクチン(MMR)､および水痘ワクチン(VAR)のキャッチアップ接種が推奨されている。また、ACIPは医師が青年および16～23歳の若年成人(16～18歳が好ましい)に対し、血清型B髄膜炎菌ワクチン(MenB)シリーズを投与することを推奨している。アメリカの若者におけるワクチン接種率を推定するため、CDCは2017年のNational Immunization Survey–Teen(NIS-Teen)データから13～17歳の20,949人の若者を分析した。2017年では、全国のHPVワクチン接種率(1回以上)は65.5％で2016年(60.4％)と比べ5.1ポイント増加し、48.6％は一連のワクチン接種が推奨されているHPV UTDで、2016年から5.2ポイント増加した。2016年から2017年の調査では、13歳までのHPV接種開始は各生まれ年で平均5.9ポイント増加し、19.6％(1998年生まれコホート)～56.3％(2004年生まれコホート)で、13歳までのHPV UTDは各生まれ年で平均3.6ポイント増加し、7.7％(1998年生まれコホート)～29.8％(2004年生まれコホート)であった。MenACWYの1回以上と2回以上接種率、MMRの2回以上接種率、VARの2回以上接種率も増加し、17歳の若者におけるMenBの1回以上の接種率は、14.5％であった。HPVの接種率とHPV UTD状態は連邦貧困水準を下回る層(各73.3％、53.7％)で、貧困水準または水準以上の層(各62.8％、46.7％)よりも多かった。HPVの接種率(1回以上)とMenACWYの1回以上/2回以上接種率は、大都市統計圏(MSA)以外で暮らす層で、MSA主要都市で暮らす層よりも低かった。HPV UTD状態およびMenACWYの1回以上/2回以上接種率は、保険未加入者で民間保険加入者よりも低かった。ワクチン接種率は管轄区によって異なリ、Tdapの接種率(1回以上)は78.9％(アラスカ州)～96.2％(マサチューセッツ州)、MenACWYの接種率(1回以上)は60.7％(ワイオミング州)～95.3％(ジョージア州)、HPVワクチンの接種率(1回以上)は 46.9％(ワイオミング州)～91.9％(ワシントンDC)であった。 HPV UTD状態は、28.8％(ミシシッピ州)～78.0％(ワシントンDC)であった。以上、HPV接種の開始はMenACWYとTdapの接種率よりも依然として低いが、HPV接種率は2013年以降、毎年平均5.1％ポイント上昇し、ワクチン未接種の10代をターゲットとし、予防接種の機会を逃さないために継続された取り組みが、他の定期的に推奨される青少年ワクチンと同等なHPVワクチン接種率をもたらしていると考えられる。
