B型肝炎ウイルス感染症の治療へのアクセス ― 全世界、2016年

2016年、慢性B型肝炎ウイルス(HBV)感染症の患者数は全世界にて2億5,700万人と推定され、うち約2,700万人(10.5％)は感染を認識し、治療は約450万人(16.7％)が受けている。WHOはHBV感染症の根絶目標を2030年としており、その達成には発症率を90％、死亡率を65％低下させるため、治療へのアクセスが重要である。HBVワクチン接種率は世界的に増加しており、2015年、5歳未満の幼児における慢性HBV感染症の有病率は1.3％と推定されているが、成人での有病率は依然として高い。低所得国、低中所得国、高中所得国および高所得国における治療率は低いが、高中所得国では22％ともっとも高い。2017年、低中所得国では合法的にエンテカビル後発品の入手が可能となり、また、中国とメキシコを除くすべての低中所得国にてテノホビル後発品の入手も可能となる。しかし、中国とメキシコでも2018年までで特許が切れる。アメリカおよびヨーロッパにおいてもテノホビルの特許がそれぞれ2017年、2018年に切れる。テノホビル後発品が世界市場に流通し、1年あたりの治療費が2004年の208ドルから2016年32ドルへ85％下落し、エンテカビルの治療費も2015年に427ドルと最低になっている。これらの抗ウイルス薬の価格低下はHBV感染症の治療へのアクセスの増加を後押しするものであり、また、B型肝炎治療におけるケアカスケードの定期的な分析はHBV根絶の目標に向けた進捗モニタリングを支援するものである。
