Interval Since Last HIV Test for Men and Women with Recent Risk for HIV Infection — United States, 2006–2016

最近のHIV感染リスクのある男性および女性における最後のHIV検査からの期間 ― アメリカ、2006年～2016年

2006年以降、CDCは13～64歳の全員に1回、HIVスクリーニング検査を受けるよう推奨しており、感染リスクが高い場合は年1回の再検査を推奨しているが、高リスク例の再検査率は低く、診断の遅れが続いている。今回、CDCは2006年～2016年のGeneral Society Surveyデータから、成人のHIV検査率および高リスク例の再検査の状況について分析した。回答率は61.3～71.4％であり、回答者を4群(1群：過去12カ月に男性パートナーがいた男性、2群：12カ月以内に男性パートナーはいないが複数の女性パートナーがいた男性、静注ドラッグを使用した男性、金銭の授受を伴う女性との性交を行った男性、3群：12カ月以内に複数の男性または女性と性交した女性、静注ドラッグを使用した女性、金銭の授受を伴う男性との性交を行った女性、4群：これらのリスクがない回答者)に分け、1～3群を最近のHIV感染リスク例とした。回答者は計15,956名、分析対象は11,688名であった。このうち検査を受けたことのある人は39.6％であり、最終検査からの経過期間(中央値)は1,080日(約3年)であった。年齢別の検査率は25～34歳(54.4％)、35～44歳(55.1％)にて高く、人種別では非ヒスパニック系アフリカ系アメリカ人または黒人にて57.4％と高かった。また、最近のHIV感染リスク例では62.2％、非リスク例では36.1％であった。最終検査からの経過期間は黒人が534日間(非ヒスパニック系白人：1,545日間)、18～24歳が332日間(25～34歳：657日間、35～44歳：1,403日間)、年間世帯収入$35,000未満では767日間($35,000以上：1,356日間)、最近のHIV感染リスク例が512日間(非リスク例：1,360日間)であった。最近のHIV感染リスク例の検査率は男性パートナーのいる男性が71.0％、他のリスクのある男性は58.0％、女性は65.9％であり、最終検査からの経過期間はそれぞれ459日間、610日間、416日間であった。以上、2006年～2016年における最近のHIV感染リスク例のHIV検査率は60.0～66.7％、最終検査からの経過期間は毎年365日を超えており、少なくとも年1回、再検査を受けるための対策を強化する必要性が示唆された。
