Access to Syringe Services Programs — Kentucky, North Carolina, and West Virginia, 2013–2017

シリンジサービスプログラムの利用 ― ケンタッキー州、ノースカロライナ州およびウェストバージニア州、2013年～2017年

アメリカのアパラチア地区では注射薬物使用(IDU)によるC型肝炎ウイルス(HCV)感染が増加している。シリンジサービスプログラム(SSPs)は安全な注射器を提供するものであり、HCV感染を56％低下、SSPsと投薬療法の併用では74％低下させるが、アメリカではSSPsの利用がとくに農村部、南部および中西部の州において制限されている。ここでは2013年～2017年8月1日におけるケンタッキー州、ノースカロライナ州およびウェストバージニア州のSSPs拡大について報告する。ケンタッキー州では2015年3月に新しい法律が制定されるまでSSPsは違法であった。公衆衛生局は新法の制定により関連する郡保健局、郡裁判所および市議会の承認後、SSPsの運用を開始、2015年末までに2大都市ルイビルとレキシントンを含む3郡、2017年8月までに31郡にて推定8,078名の利用者に対しSSPsが行われている。ノースカロライナ州では2016年7月11日に新しい法律によりSSPsの開始が認められ、2017年8月1日の時点で20回の運用により州内100郡のうち52郡にて推定3,983名に対しSSPsが行われた。ウェストバージニア州では州法による規制はなく、2017年8月1日の時点で9回のSSPsが推定4,376名に対して行われている。SSPsはこれら3州にて急速に増加しており、包括的SSPsの実施によりIDUによる健康への影響が緩和するものと思われる。
