65歳以上の人の転倒による死亡 ― アメリカ、2007年～2016年

毎年、65歳以上のアメリカ住民の4人に1人が転倒しており、それによる救急外来受診は約300万人と推計される。CDCは2007年～2016年における65歳以上の高齢者の転倒による年齢補正死亡率および死亡の傾向について、性別、年齢層、人種/民族、居住都市/地方および州別に分析した。その結果、年齢補正死亡率は2007年が18,344人(10万人あたり47.0)、2016年が29,688人(同61.6) であり、31％(3.0％/年)の増加であった。2016年の州別死亡率は24.4(アラバマ州)～142.7(ウイスコンシン州)であり、年平均変化率はメイン州にてもっとも大きく(11.0％/年)、オクラホマ州(10.9％)、ウェストバージニア州(7.8％)と続いた。また、死亡率は30州およびワシントンDCにて有意に増加、11州では有意な変化は認めなかった。コロラド州、オレゴン州およびテネシー州では増加した後、ほぼ横ばいとなり、アリゾナ州、ネバダ州およびウイスコンシン州では横ばい後に有意に増加した。ミズーリ州では2007年～2012年は減少、2012年～2016年は増加、ユタ州では2007～2012年は増加し、2012～2016年は減少した。2016年における転倒による死亡率は85歳以上(257.9)、男性(72.3)、白人(68.7)にて高く、2007～2016年にてアメリカインディアン/アラスカ先住民を除くすべての人種/民族サブグループにて増加していた。死亡率の増加は85歳以上にてもっとも大きく(3.9％/年)、65～74歳では1.8％、75～84歳では2.3％であった。以上、アメリカでは高齢者の転倒による死亡が増加しており、医療関係者は高齢者にこのリスクを認識させ、適切な予防策(筋力の強化やバランス運動、理学療法など)が必要であると思われる。
