Health and Development at Age 19–24 Months of 19 Children Who Were Born with Microcephaly and Laboratory Evidence of Congenital Zika Virus Infection During the 2015 Zika Virus Outbreak — Brazil, 2017

2015年のジカウイルスアウトブレイク中に生まれた小頭症および先天性ジカウイルス感染症の臨床検査値を示す19～24カ月齢の幼児19例の健康と発育 ― ブラジル、2017年

2015～2016年、ブラジルでは15州からジカウイルス感染確診例による小頭症の出生率の増加(出生児10,000人あたり2.8例、確診例を認めない4州では0.6)が報告され、ブラジル健康省(MOH)、パライバ州Health SecretariarおよびCDCは水頭症に関するサーベイランスに報告されたブラジル北東部で生まれた幼児を追跡し、計19例のZika Outcomes and Development in Infants and Children (ZODIAC)による調査結果を報告する。ZODIACの調査対象となった幼児は2015年10月1日～2016年1月31日生まれであり、122名が登録され、うち19例(24カ月齢未満)が出生時に水頭症を呈し、検査値にてジカウイルス感染のエビデンスを示した。19例中11例がジカウイルスIgM抗体および中和抗体陽性、8例は中和抗体陽性であった。この19例[年齢中央値22(19～24)カ月齢、男児10例、女児9例]を追跡した結果、評価時、15例(男児7例、女児8例)の頭囲が平均値より>3 SD低値であり、4例は頭囲が月齢に伴い増加し、男3例は平均値より1 SD低値、女1例は1 SD高値であった。また、11例が非熱性けいれんのスクリーニング陽性であった。保護者の報告では8例に入院歴があり(うち6例は気管支炎/肺炎)、10例に摂食、嚥下障害、13例に聴覚刺激応答障害、14例に網膜異常、11例に視覚刺激応答障害を認めた。ASQ-3(乳幼児発達検査、6カ月齢時)は14例が通過せず、15例のHINE総合スコアが40を下回り、重度の運動障害が示唆された。19例全員に少なくとも１つの障害があり、12例には3～5つの障害を認めた。以上、ジカウイルス感染症のエビデンスを示す小頭症乳児は重度の機能障害を伴い、特別なケアが必要であることが示された。
