Vital Signs: Recent Trends in Stroke Death Rates — United States, 2000–2015
バイタルサイン:脳卒中の死亡率に関する最近の傾向 ― アメリカ、2000年~2015年
National Vital Statistics Systemデータから、2000~2015年のアメリカにおける35歳以上の脳卒中死について分析した。35歳以上の成人における脳卒中死亡率は2000年(10万人あたり118.4)から2015年(同:73.3)にかけて38%低下した。年間増減率(APC)は-3.1%であり、2000~2003年:-3.4%、2003~2006年:-6.6%、2006~2013年:-3.1%と低下していたが、2013~2015年は+2.5%と増加した。年齢別では35~54歳は2006~2015にて低下したが、他の年齢層では2013~2015年は増加に転じ、また、男女ともに2013~2015は増加した(男性:+0.8、女性:+2.7%)。人種別ではアメリカインディアン/アラスカ原住民では2015年まで低下したが、他の人種では増加に転じ、ヒスパニックは2000~2013年は低下(APC:-3.6%)、2013~2015年は有意な増加(+5.8%、p=0.05)を示した。地域別では南部にて2013年に増加に転じ、2006~2013年は-3.3%、2013~2015年は+4.2%(p=0.05)であった。西部では2012~2015年にて+0.6%、北東部では2013~2015年にて+0.7%、中西部では2009~2015年にて-1.5%であった。州別では38州(75%、北東部:8/9、中西部:7/12、南部:14/17、西部:9/13)は2000~2015年にて低下、フロリダ州では2013~2015年にて10.8%/年と有意に増加した。2013~2015年の脳卒中による死亡者数は32,593例以上と推計され、リスク因子を有する症例に対する脳卒中の予防および治療介入を継続する重要性が示唆された。
